About this role

Job summary

This is an exciting opportunity to deliver tangible change in the way in which care is delivered for our local community. The role will be working across our Primary Care Network of 10 practices and 70,000 patients covering the geographical area embracing Little Horton, West Bowling, and Manningham.

Horton PCN is a large and vibrant Primary Care Network. As a PCN we share a vision for our community and those who we look after; we are committed to working more closely together so we can provide a broader range of care and services for our patients, more efficiently and in a more joined up way. We will achieve this by closer working not only with each other but also with others involved in primary and community care, including community nursing teams, adult social care and voluntary and community organisations in our area.

We have recruited a number of clinical colleagues in the PCN to help us achieve this and will be consolidating these teams further over the coming years. We have a number of great ideas that will help us realise our vision and a number of projects we would love to implement. We need some help to bring all this to fruition as all existing practice staff are working at or beyond capacity so we need someone whose role is to bring to life our PCN vision and deliver projects.

Main duties of the job

We are looking for someone with a good knowledge and understanding of Primary Care who can manage the delivery of a number of projects across all of the PCN practices. You will be self-motivated with a pragmatic can-do approach to ensure that the practices start to realise the tangible benefits that we know can be achieved quickly by working better together as PCN. You will work to develop and embed the teams and this will be a catalyst to introducing more harmonised working between our practices.

About us

This is an exciting opportunity to join this GP provider organisation encompassing GP practices across Bradford City with a total patient population of around 160,000 and representing one voice for primary care in the City area.

Further ongoing support is provided through both local and countrywide governance and peer support groups. Additional professional development in line with the individuals needs will be available with shadowing, e-learning, relevant courses/conferences and higher level training opportunities being supported.

Job Summary

The post holder will be working with Horton Primary Care Network a network of ten GP practices working collaboratively across the network area with our community partners to deliver the best possible health and wellbeing care for our local population.

This is a pivotal role, the purpose of which is to provide strategic and operational management support to the Clinical Director and PCN Board to deliver local strategic objectives.

The postholder is required to work from the PCN Head Office at Little Horton Lane Medical Centre remote working from home is not an option to ensure direct, face to face collaboration with the Clinical Director and PCN and practice teams.

The manager will be comfortable looking strategically, exploring new ways of working, suggesting options for change and being prepared to push the boundaries of established thinking.

The PCN practices have agreed to work together to deliver the NHS Long Term Plan and Network Contract DES. The post holder will be expected to deliver local strategic objectives and priorities acting as the key enabler for the Clinical Directors vision.

Key Roles and Responsibilities

Strategic management and planning

Contribute to PCN strategy; formulate objectives and research and develop ideas for future PCN organisational development

Monitor and evaluate performance of the PCN against objectives; identify and manage change

Prepare and annually update a PCN Strategic Development Plan, overseeing the implementation of the aims and objectives

Assist the Clinical Director in analysing service specification, performance indicators and data collection requirements to inform design for new PCN services

Direct Collaboration with the Clinical Director (Core Requirement)

Work very closely and intensively with the Clinical Director on a daily basis, providing direct management support, strategic advice, and operational oversight from the shared office base at Little Horton Lane Medical Centre.

Act as the Clinical Director's right hand, ensuring seamless communication and execution of PCN priorities through daily face-to-face interactions.

Provide immediate, on-site support to the Clinical Director, enabling rapid decision-making and problem-solving without the delays associated with remote communication.

Attend relevant local meetings in agreement with the Clinical Director, share views, and liaise with member practices and other agencies.

Communication & Stakeholder Engagement

Develop and maintain effective communication both within the PCN and with relevant outside agencies.

Support PCN GP practices and ensure effective two-way communication between practices and the PCN. The successful candidate will understand the specific nuances and cultures of each of the 10 practices within the Horton PCN.

Build strong relationships with stakeholders, acting as the "glue" between the practices and the PCN Board.

Represent the PCN at meetings, in the community, and to the media as and when required.

Operational Delivery & Project Management

Provide project management for the implementation of PCN initiatives.

Keep abreast of national policies relating to the PCN DES. Support local PCN developments, in line with local (ICB) and national (NHSE) requirements.

Identify opportunities where it will be beneficial to work as a PCN rather than individual practices and bring these to fruition.

Oversee the coordination and delivery of enhanced services and other service submissions on behalf of the PCN.

Oversee the administration of the PCN, ensuring a well-organised and efficient operational framework.

Support and work with practices which may be experiencing difficulties achieving PCN targets, using existing relationships to facilitate support and improvement.

Governance & Compliance

Identify and record risks and issues, developing contingency plans with service leads.

Ensure all relevant national strategies, policies, targets and frameworks are fully adhered to and implemented.

Ensure compliance with GDPR and other relevant legislation and professional information management standards such as GMC/BMA and LMC guidance.

Work with the general practice teams to ensure the practices are compliant with CQC standards.

Team & Staff Management

Co-ordinate the induction, and training of new and existing PCN staff contracted via the ARRS.

Provide management and support to PCN employed staff.

Clarify objectives and priorities for network delivery in line with operational realities, financial parameters and contract requirements.

Support the clinical teams to deliver an efficient, high level of services to practices and patients.

General

Ensure that all activities are monitored and evaluated.

Attend internal meetings as required.

Work in accordance with all policies, including the equal opportunities policy and practice.

Attend any training courses and supervision sessions as required.

Work with due regard to the Health and Safety Policies, ensuring all practices and procedures are undertaken in accordance with issued guidelines.

Undertake any other duties as may reasonably be required from time to time.

To be prepared to work flexibly, including evenings and weekends if required.

Experience Essential

Specialist knowledge of working in an administrative and business environment and its systems and practices including projects, business/strategic planning and information management Demonstrable experience of effective planning and organisational skills to deliver targets to deadlines Experience of successfully developing and implementing projects. A minimum of two years experience of working in a senior management role

Desirable

Knowledge of primary care contracting and commissioning Experience of working in primary care or the wider NHS An understanding of the specific health inequalities and demographic challenges facing the Bradford City ICB area Proven ability to work collaboratively with multiple GP practices simultaneously
