Staff Pharmacist – Long-Term Care $115,000–$145,000 per year | Full-time | Primarily Monday–Friday
Leave the retail counter behind. Join a closed-door long-term care pharmacy with no drive-thru or public-facing prescription counter, a primarily Monday-through-Friday schedule, eight-hour shifts between 8:00 a.m. and 8:00 p.m., and annual compensation of $115,000–$145,000. Retail pharmacy experience is welcomed, and long-term care experience is not required. Guardian Pharmacy of South Carolina is seeking a full-time Staff Pharmacist to join our growing Charleston team. We serve assisted-living communities, skilled-nursing facilities, and individuals with complex medication needs throughout South Carolina.
Schedule and Compensation
• Annual salary range of $115,000–$145,000, based on experience and qualifications
• Primarily Monday-through-Friday schedule
• Eight-hour shifts scheduled between 8:00 a.m. and 8:00 p.m.
• Weekend rotation of approximately one weekend every three months
• No overnight shifts
• Minimal on-call responsibilities

A Different Pharmacy Environment This position allows you to continue using your clinical knowledge and professional judgment without many of the daily pressures of a traditional retail setting.
• Closed-door pharmacy with no public-facing prescription counter

Long-term care pharmacy has its own important responsibilities and deadlines, but the work is structured differently from retail. You’ll work alongside an experienced team focused on safely and accurately serving residents and their healthcare providers.
What You’ll Do
• Review and verify new prescriptions, refills, and cycle-fill medications
• Ensure medications are prepared, packaged, and dispensed accurately and on time
• Evaluate medication orders for allergies, interactions, appropriate dosing, refill timing, therapeutic appropriateness, and legal requirements
• Exercise professional judgment when determining whether a prescription can be safely and legally dispensed
• Communicate with nurses, prescribers, facilities, and other healthcare professionals
• Answer medication-related questions and help resolve order discrepancies
• Address medication availability, inventory, and out-of-stock concerns
• Provide guidance and oversight to pharmacy technicians
• Document and help evaluate medication errors and quality concerns
• Follow all federal and South Carolina Board of Pharmacy requirements
• Participate in the weekend and minimal on-call rotations
• Support other pharmacy operations as needed
What We’re Looking For
• Bachelor of Pharmacy or PharmD degree
• Active, unrestricted South Carolina pharmacist license
• At least one year of pharmacy experience preferred
• Retail pharmacy experience welcomed
• Long-term care experience is helpful but not required
• Strong attention to detail and commitment to medication safety
• Sound clinical judgment and problem-solving skills
• Ability to prioritize and manage multiple responsibilities
• Comfort with pharmacy technology and willingness to learn new systems
• Dependable, collaborative, and able to work independently when needed

Ensure all prescriptions are filled according to order and regulations, accurately and in a timely manner and proper packaging according to facility.

Essential job functions (include the following): • Accurately prepare medications for final dispensing to the patient, including filling, order entry, and packaging • Review completed orders and cycle-fill medications to ensure prescriptions were filled correctly • Verify the legality and pharmaceutical feasibility of dispensing a drug based on patients' allergies, Incompatibilities with current medications, frequency of refills, etc. • Interpret prescription orders, to include exercising professional judgment in determining whether to dispense a particular prescription, taking into account patient safety and legal limitations. • Adhere to all Federal and State Board of Pharmacy regulations • Adhere to all applicable guidelines for compounding • Transfer prescriptions to other pharmacies as needed • Answer questions from patients, doctors, nurses, etc. • Evaluate inventory levels and place orders when below par • On exception items, look up most cost-effective product to order • Notify customer/facility if product is out of stock and when the order can be filled • Restock station supplies as needed - vials, cassettes, labels, delivery tickets, etc. • Oversee the work of all technicians and give feedback to their supervisor regarding their performance • Log all error medication occurrences • Other essential functions and duties may be assigned as needed
Education and/or Certifications: • Bachelor's Degree (Pharmacy) or PharmD. required • State Pharmacist License required without restrictions
Skills and Qualifications: • 1+ years of related experience • Advanced computer skills; pharmacy operations system experience preferred/required • Ability to work independently and deliver to deadlines • Ability to solve problems with minimal direction • Great attention to detail and accuracy • Ability to excel in a fast-paced, team-oriented environment working on multiple tasks simultaneously, while adhering to strict deadlines • Quality minded; motivated to seek out errors and inquire when something appears inaccurate • Medical consulting experience preferred
