Cypress Resource Network is looking for an Care Navigator to join our centralized intake team and play a vital role in connecting residents, clients, families, and referral partners with the right services at the right time.

In this dynamic role, you'll coordinate referrals and inquiries across Cypress Cove, Cypress at Home, and other Cypress Resource Network pathways, supporting a diverse range of services including skilled nursing, Medicare-certified home health, private duty home care, outpatient therapy, internal resident transitions, and community resources.

WHAT YOU WILL BE DOING:

• Serve as the primary intake contact for referrals and inquiries from hospitals, physicians, residents, families, community partners, and internal teams.

• Review and triage referrals using established workflows and service-line guidelines to determine the appropriate care pathway and identify escalation needs.

• Coordinate referrals and admissions across skilled nursing, home health, home care, outpatient therapy, and other services.

• Gather and verify required clinical, demographic, insurance, and financial information; identify missing information, barriers, and payer requirements.

• Support insurance benefit verification, prior authorizations, recertifications, authorization extensions, and related payer communications.

• Coordinate with clinical, scheduling, billing, operational, and leadership teams to support timely admissions, starts of care, transitions, and internal handoffs.

• Maintain accurate referral records, tracking systems, dashboards, documentation, and census updates; support daily census/referral huddles.

• Communicate proactively with referral sources, families, payers, clinical teams, and location leaders regarding referral status, delays, authorizations, and next steps.

• Assist with admission audits, file reviews, reporting, and process improvement initiatives while maintaining compliance with HIPAA, organizational policies, payer requirements, and applicable regulations.

• Provide cross-coverage during PTO, high-volume periods, vacancies, emergencies, and scheduled weekend/seven-day intake coverage as needed.

WHAT WE ARE LOOKING FOR:

• High school diploma or equivalent required; associates degree or bachelors degree in healthcare administration, business, social services, nursing, therapy, or a related field preferred.

• Minimum of two years of experience in healthcare intake, admissions, authorization, care coordination, referral management, scheduling, or a similar role preferred.

• Experience with skilled nursing, home health, home care, senior living, care transitions, payer authorization, or electronic referral platforms preferred.

• Knowledge of Medicare, managed care, insurance benefit verification, authorization processes, and relevant home health or skilled nursing documentation workflows preferred.
