Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

The behavioral health care manager is a core member of the collaborative care team, including the patient's medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The behavioral health care manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient's medical provider and, when appropriate, other mental health providers.

Required Qualifications:
• Bachelor's Degree. • Master's Degree in Social Work. • Upon hire: California Clinical Social Worker License. • 2 years Inpatient, outpatient, or home-based social work. • Demonstrated experience in counseling and group work. • Experience with screening for common mental health and/or substance abuse disorders. • Experience with assessment and treatment planning for common mental health and/or substance use disorders. • Experience with evidence-based counseling techniques.
