We are seeking an enthusiastic, motivated and experienced Practice Nurse to join our friendly and supportive multidisciplinary team at Tieve Tara Medical Centre.
This is an exciting opportunity for a registered nurse with strong primary care experience to play a key role in delivering high-quality patient-centred care. The successful candidate will provide a comprehensive range of nursing services including chronic disease management, health promotion, screening, immunisation programmes and preventative healthcare.
Working as part of an integrated healthcare team, you will contribute to improving health outcomes, supporting self-management and delivering evidence-based care to a diverse patient population.
Main duties of the job
About the Role
As a Band 6 Practice Nurse, you will:
Deliver high-quality nursing care within a primary care setting.
Undertake assessment, treatment, screening and health promotion activities.
Lead and support the management of long-term conditions including diabetes, asthma, COPD, hypertension and cardiovascular disease.
Provide cervical screening services and contraception provision (including emergency contraception).
Deliver childhood, routine and travel immunisation programmes.
Undertake a range of clinical procedures including venepuncture, ECGs, wound care, urinalysis and spirometry.
Empower patients to improve their health through education, prevention and self-management support.
Participate in clinical governance, quality improvement, audit and service development activities.
Support students and junior colleagues within the practice environment.

Knowledge Essential
Experience of Nurse led clinics. Experience in contacting and discussing test results with patients. Significant experience at Band 5 (or equivalent level) in relevant primary care or community-based environment. Insight and understanding of current NHS issues in nursing and the NHS. Interest and basic knowledge in all long-term conditions with in-depth knowledge in at least one area. Demonstrate an understanding of audit with experience of undertaking regular audits Experience of compiling procedures and clinical guidelines. Clinical skills, included and not limited to: o Venepuncture o Blood pressure monitoring o ECG o Urinalysis o Swab taking. o Cervical screening o Routine childhood immunisations o Routine Vaccines o Travel health o Contraceptive injections o Peak flow measurement and Spirometry o Nebulising o Glucometer assessment o Wound care o Ear syringing o Learning Disability reviews and assessment o Long term condition diagnosis, assessment and management including diabetes, COPD, Asthma, Hypertension, Chronic Heart Disease, Lipid management, CKD, ACOS, PAD, AF, Heart failure and NDH o Insulin initiation and titration o Maintain general tidiness and cleanliness of nurses and treatment room o Maintenance and cleaning of general equipment including defibrillator and nebulisers. o Health Promotion The post holder should also have a keen interest in developing knowledge and skills across the integrated services. o The post holder will be expected to undertake safeguarding training appropriate to their role and adhere to policies and procedures relevant to the area they work.
