We are hiring a Nurse Practitioner for a Palliative Care position providing specialized symptom management and care to patients in their homes in the Plymouth area.
What You’ll Do:
• Deliver compassionate, patient-centered palliative care, partnering with patients, families, and interdisciplinary teams to improve quality of life and support goal-concordant care • Perform comprehensive assessments, manage symptoms associated with serious illness, and develop individualized treatment plans that address physical, emotional, and psychosocial needs • Facilitate goals-of-care discussions, educate patients and caregivers on treatment options, and apply advanced clinical judgment to coordinate care and maintain thorough, compliant EMR documentation • Work independently, but never alone • Travel to patient homes within assigned territory

Nurse Practitioner Qualifications
• Bachelor of Science (BSN) degree from an accredited School of Nursing • Master of Science (MSN) degree from an accredited School of Nursing • A minimum of five (5) years of clinical experience • Hold, in good standing, a license as a Registered Nurse and Nurse Practitioner issued by the state in which you work • Experience in a community based Home Health or Hospice/Palliative Care practice is strongly preferred
