About this role

Job summary

We have an exciting opportunity for motivated and forward-thinking Clinical Pharmacists to join the Haematology Pharmacy Team within Aneurin Bevan University Health Board.

These newly established posts have been created following a successful business case to enhance specialist pharmacy services for Haematology patients across the Health Board. Successful candidates will work closely with the multidisciplinary haematology team while providing cross-site support to outpatient and inpatient haematology services.

This is an excellent opportunity for an experienced clinical pharmacist to develop advanced practice within a specialist service, helping to shape pharmacy provision as the service expands. Working alongside Consultants, Advanced Nurse Practitioners, Clinical Nurse Specialists and wider pharmacy colleagues, you will contribute to the delivery of high-quality, patient-centred care for patients with haematological conditions.

Successful candidates will be supported to lead service improvements, contribute to medicines optimisation initiatives and help deliver the Health Board's vision for a modern, innovative haematology pharmacy service.

Main duties of the job

Key responsibilities include:

Providing specialist pharmacist support to haematology outpatient services at the Royal Gwent and Nevill Hall Hospitals, and inpatient services at the Grange University Hospital. Working as an integral member of the multidisciplinary haematology team, participating in MDT meetings and treatment planning. Providing expert pharmaceutical advice to medical, nursing and pharmacy colleagues on the management of patients with haematological disorders. Supporting medicines optimisation and the safe, effective and cost-effective use of medicines. Undertaking, or developing towards, independent prescribing within agreed areas of competence. Contributing to the development and delivery of Systemic Anti-Cancer Therapy (SACT) services. Supporting delivery of business case objectives through enhanced pharmacy services across inpatient and outpatient pathways. Leading and contributing to service improvement, audit, guideline development, quality improvement and clinical governance activities. Supporting education and training for pharmacy, nursing and medical staff. Monitoring high-cost medicines, supporting value-based prescribing and working collaboratively across sites to ensure equitable access to specialist pharmacy services. Person Specification

Qualifications and Knowledge Essential

Master's degree in Pharmacy (or equivalent). Member of GPhC Postgraduate clinical specialist knowledge e.g. Diploma or MSc Evidence of CPD Non Medical prescriber or willingness to work towards qualification within first 12 months

Desirable

Achieved Advanced Practice RPS Credentialing or willingness to work towards credentialing within first 12 months of post RCP member

Experience Essential

Recent Hospital pharmacy experience including experience in clinical pharmacy practice in a ward setting Experience of monitoring financial expenditure Experience of planning audit, quality improvement or research Experience of working effectively within a multi-disciplinary team Proven ability to work independently. Experience of workplace tutoring/mentoring

Desirable

Experience of delivering training programmes/lectures Experience of successfully managing and implementing change Experience working within Oncology/ Haematology clinical practice
