About this role

Job summary

This role forms part of a newly established Clinical Co-ordination Function in Birmingham and Solihull (BSOL), aimed at improving support for children and young people (CYP) with Learning Disabilities and/or Autism (LD/A).

The Clinical Co-ordinator will lead on coordinating care and support for CYP with LD/A at risk of hospital admission, ensuring that pre-admission Care, Education and Treatment Reviews (CETRs) are completed in line with NHS England guidance.

We have two roles available, one covering the Birmingham area, and one covering the Solihull area.

For Birmingham, the post holder will sit within the Disability Intensive Care Enhanced (DICE) Team, who work to prevent avoidable psychiatric admissions for children and young people with learning disabilities and / or autism. The individual would engage directly with children and young people who are admitted to a mental health hospital, and work with networks to proactively support discharge and reduce the risks of readmission.

For Solihull, the post holder will sit with the MPOWER service, who work to prevent avoidable psychiatric admissions for children and young people with learning disabilities and / or autism. The individual would engage directly with children and young people who are admitted to a mental health hospital, and work with networks to proactively support discharge and reduce the risks of readmission.

Main duties of the job

Provide clinical leadership and coordination for CYP with LD/A at risk of admission. Hold clinical oversight of complex cases, ensuring proactive risk management, contingency planning, and admission avoidance. Monitor and report on the inpatient cohort, ensuring timely discharge planning and post-discharge support. Ensure high standards of safeguarding, governance, and adherence to national and local LD/A frameworks. Offer CAMHS inpatient reviews of treatment and support discharge planning, ensuring key stakeholder from within the service are present or referrals completed,Andinitiate escalation whererequiredto prevent delays in treatment or discharge.

Training and Qualification Essential

Registered clinical professional (e.g., RNLD, RNMH, Occupational Therapist, Social Worker, Psychologist) Current registration with relevant professional body (e.g., NMC, HCPC)

Relevant Postgraduate training

Experience Essential

Experience of working with children and young people with LD/A and complex needs Experience of admission avoidance or intensive community support

Skills and Knowledge Essential

Understanding of national frameworks for CYP with LD/A (e.g. Transforming Care, CETRs, Dynamic Support Registers) Knowledge of safeguarding, SEND frameworks, and relevant legislation (Children Act, Mental Health Act) Passionate about improving outcomes for CYP with LD/A
