We are recruiting a Recovery Navigator to join our Intensive Outreach Team (IOS) in South Wiltshire.
IOS provides short-term, intensive, psychosocial support to people being discharged from AWP Intensive Services to their GP.
Main duties of the job The IOS Recovery Navigator role is a great fit, for an applicant pursuing experience relevant to a clinical psychology field but also lends itself to anyone who is passionate about supporting those with acute mental health presentations and has some form of training/experience relevant to acute/higher risk populations.

Job description Job responsibilities Work as part of an integrated system alongside AWP Intensive Services, contributing to the delivery and ongoing development of the IOS model through collaborative practice and reflective learning, within agreed role boundaries.
Build and maintain effective professional relationships with colleagues across IOS, AWP Intensive Teams and relevant partner agencies, communicating clearly, sharing information appropriately and contributing to joined-up, person-centred support. Meet with individuals face to face, online and over the phone to provide support.
Complete support conversations with the client. This conversation is a person-centred intervention to assess what matters to a person with regard to their life, support needs, goals, strengths, and desired outcomes whilst addressing their concerns. The conversation includes the Dialog+ tool that helps to monitor outcomes.
Provide outreach, PAT-informed and personalised support to individuals face-to-face, responding flexibly to acute mental health distress, crisis presentations and fluctuating levels of risk e.g. stabilisation and grounding techniques, crisis management and effective coping skills, safety planning, identifying networks of support.
Contribute proactively to handovers and MDT discussions within the IOSAWP system, sharing observations, risk concerns and progress, and supporting shared risk formulation and crisis planning across services.
Person Specification
Experience Essential
A minimum of 6 months-1 year experience working with people with mental health needs, including risk assessing and support planning Experience of providing practical and emotional support to enable the client to take up the responsibilities of their own recovery. Proven experience of working with people with multiple needs (e.g. mental health, homelessness, alcohol/drug dependency long term conditions) Proven experience of carrying out needs and/or risk assessments and the support planning process Experience of working with a number of individuals with competing needs and priorities
Desirable
Proven experience of mental health issues and/or services, either as a user, carer or supporter. Experience being involved in processes with high-risk presentations, including risk management, safety planning, safeguarding Experience delivering support to acute populations within crisis, admission or intensive community settings, including managing complexity, co-occurring needs and heightened risk.
