About this role

Job summary

We are seeking a dedicated and compassionate Care Coordinator to join our healthcare team. The successful candidate will play a vital role in organising and managing patient care plans, ensuring seamless communication between patients, families, and healthcare professionals. This position offers an opportunity to contribute meaningfully to patient wellbeing whilst working within a supportive and professional environment. Prior work experience in GP Practice is desirable, as organisational skills are essential for success in this role.

Main duties of the job

The successful candidate will demonstrate excellent interpersonal and organisational skills as well as a good level of competency in IT.

Ideally you will be a team player who has a eye for detail and a love for admin, can demonstrate outstanding communication skills, initiative, can self motivate and can prioritise your own workload.

Knowledge of some medical terminology would be advantageous.

Coordinate patient appointments, follow-ups, and care schedules efficiently Act as a point of contact for patients and their families, providing guidance and support Liaise with healthcare providers to facilitate communication and ensure continuity of care Maintain accurate and up-to-date patient records in accordance with data protection regulations Organise documentation related to treatment plans, referrals, and prescriptions Support administrative tasks such as filing, data entry, and managing correspondence Ensure compliance with organisational policies and procedures at all times

Experience Essential

Experience of working in a GP Practice/NHS setting.

Knowledge and skills Essential

Ability to provide quality care Good interpersonal and communication skills

Desirable

Basic IT skills Knowledge of SystmOne

Qualifications Essential

GCSE grade A to C in English and Maths
