Job summary

ASpecialist Community Public Health Nurse in HealthVisitingtoprovidestrong clinicalcare andleadershipwithinan integrated skill mixed teamas part of the0-19 Universal Health Service in Blackburn with Darwen.

To lead on the delivery of the Health Child Programme 0-5yearsand defined local requirements based on the provider contract.

To provide family centred, evidence based public health service thataims to proactivelyimprove healthoutcomes for a defined caseload,populationorcommunity, reducingthe risksand impactof health inequalitiesby working in partnership with families,communitiesandother professionals.This will be achieved through delivery of the model of health visiting at acommunity,universal,targeted andspecialistlevel.Safeguarding is embedded within themodelconsideringsafeand effective managementofriskforchildren, youngpeopleand families.

More than 1 position is available. Full time and part time. Working hours are Monday - Friday 9am-5pm

Presents a positive image of the service when meeting service users and partners Demonstrates advanced communication skills in order to:

i) Deal with and diffuse complex and volatile situations.

ii) Participate in the decision making process of complex issues which may be emotive with differing professional opinions in a multi-agency arena

Presents complex issues in a professional manner either verbally or in a written format, e.g. case conferences, reports and court statements where highly emotive and contentious situations may arise

Works as the named Health Visitor for a defined caseload of antenatal mothers, children and families ensuring full responsibility for their case management based on local service contractual requirements

Plans, delivers and evaluates an innovative service using evidence based knowledge e.g. ELHT policies, SOPs, guidelines and standards and works effectively with key agencies who work with children and families

Provides a holistic assessment of the needs of families taking into account physical, psychological, social and environmental factors and develops action plans to meet those needs

Delivers targeted intensive interventions to the most vulnerable families building therapeutic relationships and addressing difficult issues within families
