About this role

Job summary

To work as part of the general practice team providing high quality, evidence-based nursing care to patients. The Practice Nurse will deliver treatment, screening, and health promotion services and support the management of long-term conditions in line with NHS guidelines and practice protocols.

Main duties of the job

Provide general nursing care including:

Injections and immunisations (including childhood and adult vaccinations)

Undertake cervical screening in line with NHS Cervical Screening Programme guidance

Support management of chronic diseases such as:

Asthma

COPD

Diabetes

Hypertension

Coronary heart disease

Carry out health checks and screening programmes (e.g. NHS Health Checks)

Provide travel health advice and vaccinations.

Recognise, assess, and refer patients with deteriorating health or safeguarding concerns

Promote healthy lifestyles including smoking cessation, weight management, and physical activity

Provide advice on diet, exercise, and disease prevention

Support self-management of long-term conditions

About us

Pickhurst Surgery is based in Bromley, Hayes. We have a growing list size of approx. 8,700 patients and are part of the Hayeswick Primary Care Network. We have an extended clinical team, which includes Clinical Pharmacists, First Contact Practitioners, a Mental Health Practitioner, working alongside the GPs (3 Partners, 2 Salaried GPs) and a Nursing team lead by the Lead Practice Nurse, supported by the Practice Manager, Reception Manager and the Admin team. The Practice advocates a friendly and positive working environment and we are looking for an equally friendly, motivated and versatile individual to join our team.

Person Specification

Qualifications Essential

Registered Nurse (RN) with current NMC registration Relevant experience working in primary care or transferable clinical experience Practice Nurse qualification, Cytology training, Immunisation and vaccination training, Chronic disease management qualifications.

Experience Essential

Experience in a GP practice setting Experience of working within a nursing team.
