Job summary

Limelight Health & Wellbeing Hub is seeking a dedicated, enthusiastic, and experienced General Practice Nurse to join our vibrant team on a locum basis. Located on the first floor of a modern, purpose-built community centre in Old Trafford, our practice provides NHS primary care services to a diverse population across the M15 and M16 postcodes.

Acting autonomously within your professional scope of practice, you will deliver a wide range of general practice nursing services combining clinical expertise in chronic disease management, health screening, and immunisations with a compassionate approach.

Main duties of the job

Autonomous Clinical Delivery: Provide face-to-face and telephone nursing care, including holistic health assessments and clinical observations.

Long-Term Condition Management: Lead and deliver structured reviews for chronic conditions such as Asthma, COPD, Diabetes, Hypertension, and Cardiovascular Disease.

Immunisation & Vaccination: Administer adult and childhood immunisations, travel vaccines, and seasonal vaccination programmes in strict accordance with national guidelines and Patient Group Directions (PGDs).

Health Promotion & Screening: Conduct cervical screening, deliver lifestyle advice, undertake health check-ups, and support disease prevention initiatives tailored to our local population.

Record Keeping & Quality: Maintain clear, accurate, and contemporaneous clinical records using practice clinical software; actively contribute to Quality and Outcomes Framework (QOF) targets and local enhanced services.

Holistic Support: Social Prescribers and a dedicated Care Coordinator addressing wider social determinants of health.

Operational Support: An experienced Medicines Management Team, Reception Team, Administrative Team, and Management Team.

Job Responsibilities

1. Clinical Care & Patient Management

Assess, plan, implement, and evaluate individualised nursing care programmes for patients across all age groups.

Initiate diagnostic processes for patients suspected of having long-term conditions and support them toward self-management.

Recognise clinical deterioration or complex presentations and refer appropriately to GPs or wider multidisciplinary colleagues.

Provide travel health risk assessments, vaccinations, and preventative advice appropriate to scope of competence.

2. Medicines & Vaccine Safety

Ensure safe storage, cold-chain maintenance, stock rotation, and disposal of vaccines and pharmaceutical stock.

Administer medicines safely via PGDs or Patient Specific Directions (PSDs), recognizing and reporting any adverse events promptly.

3. Infection Control & Safeguarding

Adhere to strict Infection Prevention and Control (IPC) measures, assisting the IPC Lead with regular clinical environment audits.

Identify safeguarding concerns regarding vulnerable children or adults and follow practice escalation protocols immediately.

4. Clinical Governance & Team Working

Participate in clinical audits, significant event analyses, and quality improvement activities to support CQC compliance and high standards of care.

Engage in multidisciplinary team debriefs, clinical meetings, and daily peer-support sessions.

Support and mentor nursing trainees, healthcare assistants, and learners attached to the practice.

Person Specification

Experience Essential

Demonstrated experience in direct, face-to-face clinical care within an NHS setting. Proven competence in administering adult and child immunisations and managing anaphylaxis/resuscitation. Experience in chronic disease monitoring and health promotion. Understanding of safeguarding standards, infection control, and confidentiality/UK GDPR regulations.

Desirable

Previous experience working within NHS Primary Care / General Practice. Familiarity with EMIS Web and Docman clinical systems. Experience working with socio-economically diverse communities and addressing health inequalities. Working knowledge of QOF and local primary care enhanced services.

Qualifications Essential

Registered Nurse with current, active NMC registration. Evidence of ongoing Continuing Professional Development (CPD).

Desirable

Recognised General Practice Nursing qualification. Certificates in Long-Term Condition Management (e.g., Asthma/COPD diplomas, Diabetes care). Accredited Cervical Screening qualification. Independent / Supplementary Prescribing qualification.

Skills & Attributes Essential

Strong communication and interpersonal skills, capable of adapting delivery to overcome cultural or language barriers. Excellent time-management and organisational skills to run efficient daily clinics. Patient-focused, compassionate, approachable, and non-judgemental manner. Ability to work autonomously while remaining an active, supportive team player.
