Job summary
An exciting opportunity has arisen for an enthusiastic and motivated Practice Nurse to join our friendly and supportive team at Albany House Medical Centre.
We are looking for a compassionate and dedicated nurse who is passionate about delivering high-quality, patient-centred care within a busy GP practice. The successful candidate will provide a range of nursing services including immunisations and vaccinations, wound care, cervical screening, health promotion, long-term condition management and general treatment room duties.
You will work closely with GPs, healthcare assistants and the wider practice team to deliver excellent care and achieve the best outcomes for our patients. Previous experience in general practice is desirable, although nurses with transferable skills and an interest in developing a career in primary care are encouraged to apply.
This role is for 35.5 hours per week, typically worked on Monday-Friday pro rata with some flexibility available by agreement. The successful candidate will also participate in our Saturday morning rota (8:00am to 12:00pm), approximately one in six weekends.

Main duties of the job
As a Practice Nurse, you will deliver a wide range of treatment room and chronic disease management services, including cervical screening, wound care, venepuncture, ECGs, childhood and adult immunisations and vaccinations, spirometry, Feno testing, COPD, asthma reviews and health promotion activities.
You will support patients with the management of long-term conditions and contribute to vaccination programmes and QOF achievement.
Working closely with GPs and the wider practice team, you will assess, treat and advise patients in line with practice protocols, maintain accurate clinical records, request pathology tests within your scope of practice, and support the smooth running of clinical services. You will also contribute to the management of clinical stock and equipment, infection prevention and control activities, and the effective use of our Anima Health digital triage system.

Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs Cytology Provide cervical screening and sexual health services as per contract requirements. Wound care / ulcer care/Doppler Removal of Sutures New Patient Medicals/Urinalysis ECGs Asthma/FENO Readings/Spirometry Venepuncture Hypertension Management Ear syringing Routine immunisations/Childhood immunisations Chaperoning and assisting patients where appropriate who are being examined by another clinician Requesting pathology tests, for example urine culture, swabs Following agreed clinical protocols with referral to senior nurses or GPs as appropriateOther tasks will include:

To support the Infection Prevention Control (IPC) Team/Lead to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the Practice Manager To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF Conduct chronic disease reviews (asthma, COPD) within your remit Ensure accurate coding and documentation to support QOF indicators Monitor and recall patients for annual reviews and follow-ups where required Support vaccination and immunisation programmes (e.g., flu, shingles) Support the team to improve QOF performance Support the Delivery of services under Enhanced Services (ES), Local Incentive Schemes (LIS), and Directed Enhanced Services (DES) within your remit Contribute to data collection and reporting for contract compliance As required, advise potential diagnosis and manage acute and chronic conditions within your remit To review patient medications to enhance compliance in accordance to own scope of practice To request pathology services and process pathology results as required To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required to maintain accurate clinical records in conjunction with current legislation To ensure read codes are used effectively To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately To assist GPs with minor surgery when required To prioritise health issues and intervene appropriately To support the team in dealing with clinical emergencies To assist vaccination programme clinics for adults and childrenThe responsibilities described above are not exhaustive and may be amended from time to time in line with the needs of the practice.
