The Chislehurst Partnership is an innovative and forward-thinking GP practice providing care for approximately 24,000 patients across three sites in the heart of Chislehurst.
We are seeking an experienced and motivated Advanced Nurse Practitioner to join our friendly, ambitious and supportive multidisciplinary team. This is an exciting opportunity to play a key role in delivering high-quality patient care whilst helping shape the future of healthcare within our community.
As a valued member of our clinical team, you will work alongside GPs, Practice Nurses, Clinical Pharmacists, Paramedics, Healthcare Assistants and our wider Primary Care Network colleagues to provide outstanding care to our patients.
Main duties of the job
The Role
As an Advanced Nurse Practitioner, you will work autonomously and collaboratively within the practice team, providing expert assessment, diagnosis and treatment for patients presenting with acute and chronic conditions.
Responsibilities include:
Assessing, diagnosing and treating patients with undifferentiated and complex presentations
Managing acute and long-term conditions
Independent prescribing in line with evidence-based practice
Same-day and urgent care appointments
Home visits where appropriate
Providing clinical leadership and contributing to service development
Supporting quality improvement and clinical governance activities
Working closely with the wider multidisciplinary team and community services
The successful candidate will work across our practice sites and play a key role in delivering safe, effective and person-centred care.

Job SummaryThe Advanced Nurse Practitioner is an autonomous practitioner responsible for the assessment diagnosis treatment and management of patients presenting with acute illness minor illness long term conditions and complex healthcare needs.
The postholder will work independently and collaboratively with the wider multidisciplinary team to deliver evidence based care and improve patient outcomes. The role includes clinical leadership service development health promotion education and quality improvement activities.
