Job summary
We are looking for an enthusiastic, motivated and experienced RMN to join our Older Adult Home Treatment Team. Our team provides a service 8-8pm 365 days a year to older adults experiencing a crisis in the context of functional mental health or dementia. We work intensively with service users in a community setting to prevent admission to a mental health ward, support discharge from a mental health ward and provide comprehensive assessment and treatment plans in the person's own home.
You would be expected to demonstrate the ability to guide, support and develop junior staff with their practice, take a lead in and contribute to service development and effectively co-ordinate and manage the day-to-day workload of the team. In addition, the role involves deputising for the Team Managers in their absence
You should be able to demonstrate your passion towards working with this client group and have the ability to assess, co-ordinate treatment and support both clients and carers. You should possess excellent communication skills and have the ability to work under your own initiative and as part of a wider Multidisciplinary team.
Main duties of the job
You will be based in either the dementia or functional sub team however you will be expected to work across both sub-teams at times. As the service is city wide and the need to respond to the needs of clients in a timely manner then it is essential that you hold a full driving licence and have access to a car.
The Older Adults Home Treatment Team provides a service 365 days a year to older adults experiencing a crisis in the context of functional mental health or dementia.
The team work intensively with service users in a community setting to prevent admission to a mental health ward, support discharge from a mental health ward and provide comprehensive assessment and treatment plans in the person's own home.
The post holder will promote clinical excellence and provide clinical advice and health promotion with nursing staff and other professionals across the Trust and Social Care services.
The post holder will work within a multi-disciplinary Older Adults Home Treatment Team (OA HTT) and core responsibilities will include:
Acting as a Lead Professional and Shift Co-ordinator Working within Care Programme Approach Comprehensive assessment of new referrals Carrying a varied caseload defined by need Delivering supervision to more junior members of staff and students Lead on and contribute to service development and improvement The post holder will be required to work in accordance with the OA HTT extended working hours.

Clinical

Act as mentor to learners, keep up to date with relevant course and curriculum developments, and maintain sign off status where applicable Participate in training relevant to role Promote insight and understanding of mental health issues to service users, carers and other agencies Contribute and lead in audit, research and evaluation as appropriate Leadership and Service Development
Take a lead in and contribute to service development and improvement Deputise for the Team Manager in their absence Effectively co-ordinate and manage the day to day workload of the team
