徵：社工 1. 執行個案管理，進行身心狀況、家庭支持與照顧需求評估 2. 辦理家屬會談、外部志工團體的聯絡及現場活動安排、環境的佈置 3. 社會資源連結與資源合作網絡 4. 規劃與執行團體活動、節慶活動及社區交流方案，方案計劃與執行成果報告文書 5. 個案服務報表、評鑑資料及行政文件製作，協助機構品質改善作業
