Job summary
To work on behalf of a federation of General Practices across Blackburn with Darwen in a variety of services to facilitate at scale working and drive quality improvement.
To act as an experienced, autonomous ANP able to work independently and in conjunction with other members of the clinical team.
Experienced caring nurse and demonstrate highly developed clinical skills and be able to demonstrate critical thinking whilst working within their professional boundaries. Working within a multidisciplinary team consisting of GPs, PAs, Clinical Pharmacists and the wider practice team to deliver excellent primary care services in and across practices and as part of integrated neighbourhood teams in BwD. They will also have a key role in supporting the PCN to deliver the Network Contract DES specifications for care homes and enhanced access requirements.
The post holder will work collaboratively to meet the needs of patients, support the delivery of policy and procedures, and provide nurse leadership as required.
In order to work at this level, NMC requirements for advanced practice must be met.
Main duties of the job
Assess, diagnose, plan, implement & evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis and/or complex needs
Examine & assess patient needs from physiological/psychological perspective, & plan care
Identify, diagnose & manage treatment plans for patients at risk of developing a long-term condition
Diagnose & manage acute/chronic conditions, integrating drug/non-drug-based treatment methods into management plan
As an independent prescriber, prescribe medication as defined by legislative framework & within scope of practice
Prioritise health problems & intervene to assist patient in complex, urgent or emergency situations, inc. initiation of emergency care
Support patients to adopt health promotion strategies that promote lifestyles & apply principles of self-care
Support & manage health needs of women presenting for family planning, cervical cytology or sexual health consultation
Assess, identify & refer patients presenting with mental health needs
Implement & participate in vaccination & immunisation programmes for adults/children
Provide nursing advice to nurses working
Work with management teams develop services for patients
Participate in MDT virtual care home ward rounds, identifying patients to be assessed in conjunction with care home staff.
Work as part of MDT to optimise care & refer/ liaise with services Inc. dietetics team, Specialist care home nurse, district nursing teams & palliative care team.

Our practices have adopted new methods of working via the extended clinical workforce which include Salaried GP's, Clinical Pharmacists, Pharmacy Technicians, Physician Associates, Care Home Pharmacists and Technicians, Assistant Practitioners, Care Co-ordinators, Social Care Link Workers and Nursing Teams.
The successful candidates will be well supported in their Primary Care roles by accountable GPs and others in the PCN MDT team.

Clinical practice

Attend any training courses which are identified as being key to current practice.
