Central London Community Healthcare NHS Trust provides a wide range of high quality community services, and we're excited to welcome a Band 7 District Nurse Team Lead to our Hertfordshire Division.
This is a fantastic opportunity for an experienced, compassionate, and motivated registered nurse who is passionate about delivering excellent community nursing care and supporting colleagues to thrive.
The successful post holder will provide leadership and line management to a designated community nursing team, with responsibility for the assessment, management, and evaluation of evidence-based nursing care to patients in their own homes.
Supporting the day to day management, performance and development of the team, you would promote effective teamwork within the wider Primary Health Care Team, promoting effective communication and appropriate delegation, to the benefit of the patient and service delivery.
Monitoring the quality of care provisions and identifying areas for service improvement and development. Supporting Clinical and operational leads and Heads of Service in the delivery of high quality care. Partnership working with key stakeholders ensuring high quality, integrated and seamless care is delivered.

Main duties of the job
Duties include:
o To provide skilled, effective and evidence based nursing care to patients with complex care needs, in a variety of community settings.
o To act as an autonomous nurse taking on-going responsibility for the assessment, planning, evaluation and implementation of nursing care to individuals, groups of patients and their families on the district nursing caseload, delegating activities as appropriate.
o To undertake comprehensive assessment of the physical, psychological and social care needs of patients using the Single Assessment Process. Including the needs of carers.
o To act as the patient's advocate and to facilitate the patient's own choices with regard to nursing care, promoting independence and self-care as appropriate.
o To work in a collaborative manner with Primary Care staff, Acute services, Social Services and other agencies to ensure a seamless delivery of care.
o To provide information and support to patients, relatives and other carers that promotes and optimises positive health.
o To show confident leadership and decision making, alongside compassionate communication. With the ability to support others to learn and develop.
As this is a community-based role, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

Education/Qualification Essential
Registered Nurse with current UK NMC registration Evidence of continuing professional education and training at degree level Qualification in team / service management or prepared to work towards. Mentor or practice based learning training Community or Independent Nurse Prescribing qualification or prepared to work towards
Desirable
Post - registration qualification in district nursing or equivalent
Experience Essential
Evidence of recent post-registration nursing experience in community nursing Experience of negotiating and liaising within a multi-disciplinary team Experience of working with people with complex needs Experience of co-ordinating and managing a team Experience of teaching others and mentoring students Experience of evidence-based care Experience of undertaking comprehensive health needs assessment and developing complex care plans Experience of co-ordinating and delivering care for people with complex, long term conditions.
Skills & Knowledge Essential
Knowledge of principles of care for long term conditions, relevant NSFs and other national standards Knowledge of NMC Code of Professional Conduct. Knowledge of Personal Development Planning. Knowledge of current health care policy and relevance to the delivery of healthcare services Detailed knowledge of Primary Health Care and collaborative working with other care providers, e.g. social care and voluntary services Knowledge of clinical governance & clinical supervision Knowledge of clinical governance & clinical supervision Able to work autonomously, planning and prioritising own workload and that of others Has a range of clinical skills and expertise relevant to the role Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
