Job summary
As a Senior Clinical Nurse, youll provide safe and effective clinical care to patients transitioning between hospital and community settings. Youll work closely with NHS hospitals, healthcare professionals, patients and carers to ensure a joined-up approach to care.
Main duties of the job
Alongside maintaining your own clinical practice, you'll lead and support a small team of Clinical Nurses, helping them deliver excellent patient care and achieve agreed service standards.

Your responsibilities will include:
Delivering clinical care and patient education in patients homes and virtuallySupporting patients, families and carers throughout their treatmentWorking with NHS hospitals and multidisciplinary teams to support safe discharge and ongoing careLeading, coaching and supporting a small clinical teamMonitoring clinical quality, patient outcomes and service KPIsMaintaining accurate and timely clinical documentationManaging and escalating clinical incidents and adverse events appropriatelySupporting training, development and performance within the teamParticipating in the on-call service as required
Person Specification
Experience Essential
To be successful in this role, you will have: Current NMC registration Minimum 2 years post-registration experience Demonstrable leadership or mentoring experience Excellent communication and interpersonal skills A strong patient-focused approach Full UK driving licence and willingness to travel across your region
Desirable
Desirable: Experience with CVAD, parenteral nutrition, or similar therapies
Qualifications Essential
NMC Registered Nurse with 2+ years post-registration experience
