About Us
The Prince Edward Family Health Team (PEFHT), funded by Ontario Health, offers comprehensive, multidisciplinary, and patient-centred care for Prince Edward County residents and surrounding area residents. Services and programs include disease diagnosis, treatment and management, prevention and cure, rehabilitation, palliative care, and community health promotion.
The Role
Within the context of a patient-centred care model, and working within a multidisciplinary team, the Exercise and Rehabilitation Specialist:
• Works with patients to set and achieve functional goals as applicable.
• Develops in-home and group-based exercise programs that prevent injury and provide rehabilitative and therapeutic benefits for patients whose needs require an exercise program that incorporates specific attention to underlying medical issues and associated risks.
• May develop, coordinate and deliver falls prevention education for the senior population.
• Provides complete, relevant assessments and re-assessments of patients and their environment as applicable utilizing pre-set procedures as applicable.
• Implements kinesiology strategies or interventions according to patient needs as appropriate.
• Describes and documents kinesiology processes clearly, concisely, accurately and appropriately, according to regulatory and Ontario Health standards.
• Utilizes appropriate interviewing and communication skills to obtain information from the patient as applicable.
• Promotes involvement of patient, family or caregiver in determining health and service needs as appropriate.
• Analyses and incorporates current research findings into practice.
• Documents rationale for referrals based on relevant, sound data and established guidelines.
• Identifies and utilizes tools and resources for relevant work and manages these resources.
• Works collaboratively within multidisciplinary team and acts as a clinical resource and mentor for other team members.
• Works in partnership with community agencies and programs to support patient needs.
• Promotes patient independence through teaching and health promotion.
• Promotes a safe and healthy workplace ensuring workplace conduct and activities are in accordance with the provincial Occupational Health and Safety Act and Regulations and compliant with PEFHT policies and safe work practices and procedures
• Works in collaboration with other staff in a team approach to service delivery
Qualifications
• Bachelor's degree in Kinesiology, Exercise Science, Human Kinetics, or a related field.
• Certification from a recognized fitness association (e.g., CSEP or OFC)
• Minimum 2 years related experience in the assessment and treatment of patients/clients.
• Current BCLS and 1st Aid certification
• Experience with rehabilitative exercise techniques and modalities
• Ability to design individualized rehabilitation programs
• Knowledge and experience in program planning and evaluation.
• Demonstrated knowledge of the social and health care services network and community resources.
• Ability to work collaboratively within an interdisciplinary team, as well as to work independently.
• Able to initiate and support health teaching, facilitation, and development.
• Commitment to providing an excellent patient experience.
• Valid driver’s license and proof of vehicle insurance, access to a reliable vehicle
• Satisfactory Vulnerable Sector Screening Police Check
