JOB DESCRIPTION
Job Summary
This role will be responsible for ensuring compliance with the State of Michigan Medicaid population health contract requirements, managing, tracking and monitoring CBO relationships, developing and disseminating collateral to support chronic condition management, data analysis, and development of strategies to address health related social needs. Your contributions will be instrumental in making business decisions that aim to improve the health and wellbeing of Michigan residents.
The ideal candidate will be an experienced leader who has demonstrated success collaborating and working across health plan departments to design and implement population health programs. This role utilizes market research and analysis to drive decision-making.
Job Duties
• Designing and implementing population health programs to ensure Molina meets the needs of members and communities across Michigan, while maintaining compliance with State contracts • Responsible for ensuring well-documented policies, workflows, program controls, internal and third-party practices, playbooks and best practices for respective program. • Collaborates with Legal, Compliance, and Information Security to ensure governance standards are upheld. • Tracks performance metrics and ensures value realization from deployed solutions. • Coordinates recurring meetings to support governance framework and decision-making processes, as needed. • At the direction of program (CoE, Shared Service or other functional area) leadership, supports portfolio management and/or initiative-specific change and project management. • Collaborates with key stakeholders to support dissemination and adoption of program guardrails, processes, best practices and other collateral. • Routinely reviews program collateral to ensure current and accurate reflection of business needs. • Identifies opportunities/gaps and provides recommendations on program enhancements to respective leadership team. • Responsible for creating business requirements documents, test plans, requirements traceability matrix, user training materials and other related documentations. • Generates and distributes standard reports on schedule.
JOB QUALIFICATIONS
REQUIRED QUALIFICATIONS:
• At least 4 years of Program and/or Project management experience, or equivalent combination of relevant education and experience. • Operational Process Improvement experience. • Managed Care experience, preferably in a shared service, CoE or matrixed environment. • Experience with Microsoft Project and Visio. • Strong presentation and communication skills.
PREFERRED QUALIFICATIONS:
• Experience with government-sponsored programs (Michigan Medicaid, Medicare, Marketplace). • Familiarity with value-based care and alternative payment models, community health needs assessment, quality improvement, and social influences of health and experience with practical application of those concepts • Experience in quality improvement and provider engagement strategies to improve population health. • Experience evaluating program impact/ROI to inform decision making. • Experience developing relationships with multidisciplinary teams, key providers, Community Based Organizations (CBOs), and healthcare organizations
