The Navigator for the SMMT Grant is a key member of the multidisciplinary team, providing comprehensive support to clients throughout their pregnancy and up to one year postpartum. This role involves coordinating subspecialty appointments,community outreach, and offering continuous support and resources. Acting as the primary contact, the navigator addresses client needs, maintains communication with referringclients, and serves as a daily liaison between community resources and clietns. Additionally, the navigator participates in community outreach projects to promote maternal health. Administrative duties include maintaining client files, managing reports and documents, coordinating meetings, and handling calls and vinquiries with excellent etiquette. This position is essential for ensuring seamless care and support for mothers, contributing to the success of the SMMT Grant.
Education: ASSOCIATE DEGREE/BACHELORS PREFERRED
Experience: 2-3 YEARS
License REGISTERED NURSE
Certification BLS/CPR (AMERICAN HEART ASSN)
