Job summary

Join Procare Community Services as a Community Matron and play a pivotal role in delivering proactive, person-centred care to adults with complex long-term conditions, frailty, and multiple health needs. Working within an Integrated Neighbourhood Team, you will provide advanced clinical assessment, case management, and leadership to support people to remain independent at home and reduce avoidable hospital admissions. You will work collaboratively with GPs, community services, social care, mental health teams, and other partners to coordinate holistic care and improve outcomes for patients and carers. Main duties of the job

The post holder will manage a caseload of individuals with complex needs and long-term conditions who are identified as being at high risk of unnecessary admission to hospital. You will need to have the nursing skills, experience, and expertise to assess, diagnose, treat, and evaluate health and needs.All community nurses use EMIS as the clinical record system and are provided with a work Ipad and Iphone. A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Person Specification

Qualifications Essential

Registered Level 1 Nurse with current Nursing and Midwifery Council registration Qualification or experience equivalent to MSc / level 7, including health assessment module Assessor / NMC Supervisor / Mentorship

Knowledge and Experience Essential

Advanced clinical assessment and decision-making skills, including the recognition and management of deterioration, frailty, and long-term conditions. Experience of managing complex caseloads and coordinating care for patients with multiple long-term conditions and complex health needs. Knowledge of safeguarding, risk assessment, health and safety, and clinical governance principles.
