Patient Pathway Coordinator (Band 4) - Scheduling Team (eRS)
The Scheduling Team Patient Pathway Coordinator will play a key role in managing outpatient referrals and appointments through the NHS e-Referral Service (eRS), ensuring patients are booked efficiently and in line with Trust access standards. The post holder will be responsible for processing referrals, managing appointment outcomes, validating waiting lists, and supporting the effective use of clinic capacity. Applicants should have experience of working with eRS and patient administration systems, alongside a strong understanding of referral management processes, excellent communication skills, and the ability to work collaboratively with clinical and administrative teams to deliver an efficient, patient-focused service.

Main duties of the job
Key duites include:
Monitor and review patient care and/or treatment progress using the Patient Tracking List. Identify from the Tracking List the next steps in the patient care pathway in line with clinical need (peer review). Receive patient referrals and administer in line with timings and requirements as set out in Trust and National policies Provide day-to-day supervision and oversight of duties for the department's administrators. Providing support for day-to-day queries from administrators. Responsible for collecting and updating patient information on the relevant databases in line with local policy,
