Job summary

Bretton Park Healthcare is looking to recruit an enthusiastic and motivated Care Coordinator / Executive Assistant to join our busy and friendly practice team.

This is an exciting opportunity to work within a modern GP practice environment, supporting patients to access the right care while providing excellent administrative and reception services. The successful candidate will work across both our Bretton and Park sites as part of a supportive and experienced team.

Main duties of the job

This is a dual role combining Care Coordination and Executive Assistant support within the practice.

Support patients with appointments, enquiries, prescriptions, and access to health and social care services.Coordinate care for vulnerable patients and work with clinicians, community services, and healthcare providers.Provide administrative support to the senior management team, including diary management, meetings, agendas, and minutes. Manage shared email inboxes, correspondence, reports, and follow up actions from meetings.

Care Coordinator/ Executive Assistant Job Description

Job Title: Care Coordinator/Executive Assistant

Reports To: Senior Management Team

Hours: 37.5 22 hrs Exec Assistant, 15 hrs CC You will have scheduled time for both roles

Salary: 25 27 K

Location: Bretton Park Healthcare

Main Duties and Responsibilities

As a Care Coordinator / Executive Assistant, you will play a key role in supporting the senior management team acting as key point of contact for leadership support, patients and clinicians by:

Reception and Patient Services

Welcoming patients and visitors in a professional and friendly manner. Answering telephone calls and responding to patient enquiries. Booking, amending and cancelling appointments. Processing online patient requests and enquiries. Registering new patients and updating patient records. Processing repeat prescription requests in accordance with practice procedures. Signposting patients to the most appropriate service or healthcare professional. Care Coordination

Supporting patients to navigate health and social care services. Assisting patients in managing appointments and accessing appropriate support. Liaising with clinicians, community services, care homes and external healthcare providers. Supporting vulnerable patients and those with complex health needs. Working alongside Social Prescribing Link Workers and other members of the multidisciplinary team. Following up patient actions and ensuring continuity of care where appropri Executive Support

Provide business support as directed by the senior management team (these tasks will vary but include preparing recruitment packs, copying support information

for meetings)

Manage diaries and meetings.

Prepare agendas, supporting documents and minutes.

Draft correspondence and reports.

Monitor the management generic email box daily actioning and forwarding on as required.

Be the practice point of contact for the Bretton and Park generic email boxes.

Monitor actions from minutes of meetings chasing updates when required Keeping the practice action log updated and RAG rated, give update at business meetings.

Person Specification

Qualifications Essential

Excellent communication and interpersonal skills. Strong organisational, administrative, and IT skills. Ability to manage diaries, meetings, correspondence, and action tracking. Excellent customer service skills with a professional and patient-focused approach. Strong attention to detail and ability to maintain confidentiality. Ability to work independently and collaboratively within a team. Good understanding of GP practice operations and NHS services.

Desirable

Previous GP practice or healthcare experience. Previous reception or customer service experience. Knowledge of SystmOne. Experience using Microsoft Office applications.
