Come and Join Our Caring Practice TeamWe are looking for a compassionate and enthusiastic Practice Nurse to join our friendly GP practice for 16 hours per week.

Main duties of the job

You will play a key role in supporting the health and wellbeing of our patients by:

Delivering a range of nursing services including wound care, cervical screening and immunisationsSupporting patients living with long-term conditions such as diabetes, asthma, COPD, and hypertensionProviding health promotion and lifestyle advice to help patients improve their overall wellbeingUndertaking clinical procedures including venepuncture and ECGsMaintaining accurate clinical records and contributing to the delivery of safe, effective careWorking closely with GPs and colleagues to provide the best possible patient experience

Job description Job responsibilities

Your responsibilities will include:

Delivering evidence-based nursing care within the practice setting.Managing and supporting patients with long-term conditions including diabetes, asthma, COPD, hypertension, and cardiovascular disease.Undertaking chronic disease reviews.Providing cervical screening, immunisations, vaccinations, travel health advice, and wound care services.Supporting patients with lifestyle changes and delivering health promotion and wellbeing initiatives.Identifying patients at risk of developing long-term conditions and providing proactive interventions.Supporting patients with medication management and self-care.Maintaining accurate clinical records and contributing to quality improvement programmes.Working collaboratively with the wider multidisciplinary team and external healthcare professionals to ensure coordinated patient care.Supporting safeguarding, infection prevention, and clinical governance activities within the practice.

Person Specification

Experience Essential

Wound care/removal of sutures and staples ECGs Venepuncture Requesting pathology tests and processing the results, advising patients accordingly Understanding the importance of evidence-based practice Understand the requirement for PGDs and associated policy Ability to record accurate clinical notes Ability to work within own scope of practice and understanding when to refer to GPs Chaperone procedure Women's health (cervical cytology, contraception, etc.) Immunisations (routine, childhood and travel) Chronic disease management

Desirable

Spirometry Broad knowledge of clinical governance Knowledge of public health issues in the local area Awareness of issues within the wider health arena Knowledge of health promotion strategies

Qualifications Essential

We are looking for someone who: Is a Registered Nurse with current NMC registration. Has experience in general practice nursing or a strong interest in primary care. Is confident in delivering high-quality, patient-centred care. Has excellent communication and interpersonal skills. Enjoys working both independently and as part of a team. Is committed to continuous professional development and service improvement. Shares our values of kindness, teamwork, and putting patients at the heart of everything we do.
