About this role

Job summary

The Specialist Nurse will be required:

To liaise with all key members of the multidisciplinary team to advise and support colleagues in the delivery of specialised patient care across professional boundaries. To work autonomously to deliver specialist clinical care appropriate to the needs ofthe patient group and based on principles that are research based and whichenhance the quality of patient care. To contribute to the facilitation of the planning, delivery and evaluation of carepathways for both outpatients and in- patients with Diabetes. To be responsible for the care of a case load of patients requiring specialist nursingcare within the outpatient MDT and Nurse led clinics under the supervision fromband 7/8a DSNs. Providing holistic & psychological support, advice and information to diabetespatients and their relatives and carers both face to face and using our diabeteshelpline service and online email. To support staff and patient education, have an understanding of insulin pump &CGM services. This will require excellent IT skills. To provide specialist support to the ward areas. To support the nursing team to monitor the safety and quality of care for patients. To ensure a positive focused service incorporating patients' feedback tomaintain and improve patient care. To act as an effective role model promoting excellence through practice.

Main duties of the job

CLINICAL RESPONSIBILITIES

Maintain a professional behaviour at all times and promote a positive image ofRoyal Free London NHS Foundation Trust at all times, in line with World ClassCare values. Ensure that the needs of patients are placed at the centre of care delivery, thusachieving trust and national standards for clinical quality, safety and patientexperience. To participate in and manage caseload of inpatients with the support of InpatientClinical Nurse Specialists, Lead nurses and Consultants. To use advanced clinical assessment skills and judgement to assess, plan,implement and evaluate care for patients with complex physical andpsychological problems using a recognised, evidence base model of care. Suggest and interpret relevant tests and investigations. Provide appropriate and timely education and advice to patients on all treatmentoptions available. To act as a clinical expert role model in nursing care to other nurses withregards to Diabetes.
