About this role

Job summary

We are seeking two experienced and forward-thinking clinical pharmacists with advanced expertise in frailty, multi-morbidity, and polypharmacy to lead medicines optimisation for older adults across Lambeth and Southwark. As Guy's and St Thomas' NHS Foundation Trust hosts the pharmacy service for local care homes, these roles provides both strategic oversight and expert clinical leadership for a system-wide service spanning the full continuum of frailty care, from acute hospital services to community pathways, care homes, and primary care, ensuring seamless and consistently high quality medicines optimisation for older adults wherever they receive care.

These positions offer the opportunity to work closely with the Lead Pharmacist for Care Homes and Frailty and the Consultant Pharmacist for Older People. The post holders will benefit from structured mentorship, shared leadership opportunities, and support to grow as expert practitioners and influential system leaders.

These roles require two highly motivated clinicians with strong leadership skills, a passion for innovation, and the ability to collaborate effectively across multidisciplinary teams.

Main duties of the job

As a Specialist Care Homes & Frailty Pharmacist, you will deliver high-quality, person-centred medicines optimisation for older adults living in care homes and those with frailty across Lambeth and Southwark. You will carry out structured medication reviews, develop care plans, support safe prescribing and deprescribing, and work closely with multidisciplinary teams to reduce medicines-related harm and improve outcomes.

You will provide specialist clinical advice, strengthen continuity of care across hospital, community and primary care, and contribute to service development through guideline writing, audit, training and quality improvement. As a senior member of the Care Homes & Frailty Pharmacy Team, you will contribute to leadership, management and mentorship, supporting the development of colleagues within the team and across wider system partners, while promoting best practice in medicines optimisation for older people.

Person Specification

Qualifications Essential

Current registration with the General Pharmaceutical Council (GPhC) Postgraduate Diploma, MSc or equivalent advanced clinical qualification in Pharmacy Practice

Desirable

Independent Prescriber qualification Additional qualification/training in care of older people, frailty, geriatric medicine or long-term conditions

Experience Essential

Delivering direct pharmaceutical care to patients with long-term conditions including multimorbidity and frailty Experience supporting safe prescribing and deprescribing initiatives Experience of clinical audit, quality improvement and service evaluation Experience delivering education and training to health and care professionals Undertaking clinical medication reviews in frail, older people and improving adherence

Desirable

Experience working in primary care, community health services or Integrated Care Systems Experience of working in care homes or with other social care providers

Knowledge Essential

Understands policies and the context around managing long-term conditions, case management and reducing unnecessary hospital admissions in the NHS and their implication for medicines management Understands the effects of ageing on drug handling and medicines use and understands polypharmacy and deprescribing principles Knowledge of NHS priorities relating to frailty, ageing well, patient safety and reducing avoidable admissions Understanding of primary, community, social care and acute care interfaces and transitions of care

Knowledge of prescribing data analysis and population health approaches
