Position Summary:

Unison Health is seeking a full-time Care Navigator to support clients at the Cherry Street Ministry Mission. This position manages an assigned caseload and assists clients with securing and maintaining stable housing, accessing community resources, and overcoming barriers to independence. The Care Navigator will provide case management services, monitor housing stability, maintain client confidentiality, and participate in team meetings, supervision, and required trainings.

Key Responsibilities & Role Highlights:

• The Navigator will be placed within the Cherry Street Ministry Mission.

• The position works to assist clients (assigned caseload) to secure housing within the community and assists in monitoring their placement to ensure stability of housing.

• Performs case management duties, assisting clients with overcoming barriers of obtaining and maintaining resources. Maintains confidentiality and ensures release of information is handled in accordance with federal guidelines and agency policy/procedures.

• Participates in team meetings, supervision, in-service training, and conferences according to established policies, proce­dures, and applicable regulations.

Education & Experience Requirements:

• High school diploma required. Four-year degree in social work, psychology, or related field preferred.

• Minimum of a CDCA license preferred. LCDC II/III or LICDC preferred.

• Must be able to work a flexible schedule that may occasionally include evenings and weekends.
