Dietitian Float Rate: $36- $38 (depending on experience) Shift: Per-Diem
Taconic Rehabilitation & Nursing is looking to add a Dietitian to its team. In this role, you'll help ensure residents receive individualized nutritional care that supports their health, well-being, and quality of life. RESPONSIBILITIES:
• Complete and oversee nutritional assessments for newly admitted residents and develop individualized nutrition care plans
• Monitor resident weights, meal intake, supplements, hydration status, laboratory values, and therapeutic diet needs
• Collaborate with the Food Service Director, Diet Technician, nursing staff, therapists, and other disciplines to ensure appropriate nutritional care
• Provide nutrition counseling and education to residents, families, and facility staff
• Review menus, recipes, dietary consistencies, and nourishment programs to ensure nutritional adequacy and regulatory compliance
• Participate in care plan meetings, interdisciplinary meetings, and quality improvement initiatives
• Maintain required documentation, transfer/discharge summaries, and nutrition-related records
• Assist with policy review, dietary education programs, and maintaining the facility diet manual
• Perform additional duties as assigned
REQUIREMENTS:
• Minimum education requirement: Associate's Degree in Dietetics/Nutrition; Bachelor's Degree in Dietetics/Nutrition preferred
• Minimum experience requirement: Ten full-time equivalent years of education and supervised experience (1,600 hours of experience equals one year of credit for every 30 semester hours of education); completion of Bachelor's program experience requirements preferred
• Valid NYS Dietitian License required
• Strong knowledge of nutritional assessment, care planning, hydration management, therapeutic diets, and food service operations
• Excellent communication, education, and interdisciplinary collaboration skills
• Strong organizational, leadership, and problem-solving abilities
• Ability to maintain regulatory compliance, confidentiality, and professional standards
