Job Summary:
Fulltime days travel based within Wellstar community /territory to home health patients as needed.
32 hrs /week - four 8 hour shifts primarily Monday-Friday with some flexibility/sensitivity based on community needs /staffing support.
Territory can run throughout territory and normally around 6 patients a day- home health functions focused on showering/bathing.oral care and shorter time in each home to manage 6 patients per day with quality care.
Caring/nurturing personality/empathic to family and individual care - Wellstar standards of care and compassion and committment to the individual.
Great opportunity to grow and develop = must have one year clinical and valid CNA license and preferably field experience with a reliable motor vehicle ( mileage reimbursed)
The Home Health Aide functioning under the direction of the Manager provides direct hands on-personal care and is responsible for age appropriate and culturally aware interventions of the patient, including physical, emotional, developmental and psychological care of the adult and older adult patient and family as appropriate. The Home Health Aide provides basic care needs (eg. Hygiene, nutrition, elimination, physical comfort measures) as assigned. The Home Health Aide is responsible for communication of any patient/family specific needs or concerns to other members of the team. The Home Health Aide is responsible for coordinating and collaborating with the other disciplines providing care to the patient, in addition to the patient schedule.
Core Responsibilities and Essential Functions:
DIRECT PATIENT CARE Patient Centered Care: Supports an environment of comfort and support for patients which includes: addressing patient needs in partnership with the patient; following the plan of care as assigned or instructed by a licensed professional; and while carrying out role responsibilities, engages the patient and family in compliance with the treatment plan. Effectively contributes to the plan of care or treatment intervention based on assigned tasks and patient/family input in collaboration with the healthcare team with a focus on the care continuum. Consistently implements all safety interventions to minimize risk in achievement of intended outcomes. * Provides personal care and assistance with activities of daily living per written home health aide care plan * Utilizes resources and supplies in order to provide cost effective care. * Observes patients for changes in condition and/or environment and reports to RN or Therapist * Meets established productivity standards * Seeks training, supervision or direction from leadership when appropriate COMMUNICATION AND DOCUMENTATION * Responsible for accurate completion of required documentation within established timeframe. * Demonstrates knowledge and appropriate application of regulatory guidelines * Participates in care team conferences with coworkers * Responsible for checking voice mail box a minimum of once daily. * Follows established protocol for checking in and out. COORDINATION OF SERVICES * Timely notification of supervisor/scheduler regarding all visit refusals and cancellations. * Timely notification of patient/caregivers regarding scheduled visits. * Perform other duties as assigned by Home Health Leadership STAFF EDUCATION AND PROFESSIONAL DEVELOPMENT * Responsible for attending continuing education and in-services as directed and needed * Responsible for maintaining current knowledge and skills to provide age appropriate care to adult and older adult patients. Performs other duties as assigned Complies with all WellStar Health System policies, standards of work, and code of conduct.
Required Minimum Education:
• High School Diploma / GED General
Required Minimum License(s) and Certification(s):
All certifications are required upon hire unless otherwise stated.
• CNA - Cert Nurse Assistant • BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor • GADL - Georgia Driver's License
Additional License(s) and Certification(s):
CPR Upon Hire Required
Required Minimum Experience:
Minimum 1 year experience in direct patient care clinical setting setting. Required and Home health experience Preferred
Required Minimum Skills:
Excellent organizational and time management skills. Excellent written and verbal communication skills. Ability to write and follow directions to patients house. Use of Windows based computer, utilizing home health patient software, email, internet, computer based learning modules, etc.
