About this role

Job summary

These roles are central to leading and supporting the development of Integrated Neighbourhood Teams (INTs) that bring together professionals from across health, social care and the voluntary sector to provide joined-up, person-centred care focussing first on our priority population groups - Children and Young People, Frailty or Multiple Long-Term Conditions.

We are seeking individuals who will lead collaboratively across organisational boundaries, embrace flexible and innovative ways of working, and commit to continuous improvement. This represents a significant cultural shiftmoving from a single-organisation focus to being part of a truly integrated team that places residents and communities at the heart of decision-making.

Successful candidates will champion and lead co-production, develop and maintaining strong relationships with local partners, and help shape services for the population group whilst reducing health inequalities and improving outcomes for all.

Main duties of the job

Lead and foster collaboration between primary care, community health, mental health, social care, wider local government services and the voluntary sector to deliver coordinated care in the neighbourhood to better meet residents needs and what matters to them

Provide neighbourhood leadership to the creation of seamless "team of teams" within the neighbourhood

Support the three shifts in the NHS 10-year plan

Work in partnership with secondary care provider neighbourhood leads, the neighbourhood manager and team to lead, develop, implement, deliver and embed neighbourhood working in one Southwark neighbourhood

Provide leadership, advice and support to INT system colleagues

Provide clinical leadership to all INT staff working in a neighbourhood by

Work in partnership with the INT manager provide operational and professional management to staff working as part of the integrated neighbourhood scheme,

Reach into and work with local communities and patients to ensure neighbourhood services are responsive to their needs.

Champion neighbourhood outreach service for underserved communities or those with greater complexity to reduce inequity of access and outcomes.

These roles will provide 2 sessions per week in each neighbourhood - fixed term for 12 months. They will report to both the Southwark Primary Care Provider Alliance and the Southwark Integrator Delivery Board.

Person Specification

Criteria for Primary Care Neighbourhood Clinical Directors Essential

Currently working at least 3 - 4 primary care sessions in Southwark Registered clinical professional (e.g., doctor, nurse, allied health professional) prescriber Experience of successful leadership and improvement across organisations to improve outcomes for patients and out populations Experience of successful team management across organisations and professional groups Deep understanding of integrated care systems, partnership delivery and population health Experience of teaching, mentoring, or supervision of clinical and non clinical staff
