Social Worker-PRN Who we are: Formerly known as Towanda Memorial Hospital, this facility is a satellite campus of Guthrie Robert Packer Hospital, located 17 miles north in Sayre, Pa. The Towanda campus joined Guthrie in April 2015, serving Bradford and Sullivan counties, offering ease of access to clinical services to those in the surrounding communities. Long-term care services are also provided for patients with a 68-bed skilled nursing unit and a 94-bed personal care home. How you'll contribute The Social Worker. assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems. Evaluate patients' ability to safely perform daily tasks and create personalized care plans tailored to their goals and challenges. Essential Functions
• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives. • Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions. • Participate in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan. • Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan. • Supervises and/or trains new staff, students, and interns. • Provides individual, family and group therapy. What we're looking for Education: Bachelors required; master’s preferred Licenses: Board Certified Social Worker, or related field (i.e. counseling) in the state of Kentucky (CSW, LPCA, LCSW) within 6 months of hire Certifications: Handle with Care within 30 days of hire.
