The Shrewsbury South Community Nursing Team has a vacancy for a Community Healthcare Assistant (HCA).

Under the supervision and support from the team you will be required to visit patients in their own homes providing clinical care.

We are seeking a highly motivated, enthusiastic individual with good communication skills and a passion for caring for patients in their own homes.

There is an expectation that the successful candidate will undertake our in-house development program for extended skills which will involve insulin administration, injections and wound care.

Main duties of the job

Promotes a 'can do' attitude and determination to develop and sustain a dynamic and responsive administrative and operational service within a community setting which delivers best practice. Able to implement interventions, liaising with colleagues and partners to ensure an integrated approach to the needs of the service user. To organise and deliver skilled clinical support work without direct supervision. To manage clinical risk within delegated patient caseload, referring to the supervising clinician for support and supervision.

Qualifications and Training Essential

Good general level of education (to include Maths and English) Equivalent relevant experience to vocational qualification level 3 Completion of the Care Certificate

Experience Essential

Awareness of the role of the Healthcare Support Worker

Knowledge and Skills Essential

Knowledge of the role of the health care assistant role in context, including phlebotomy and wound care, and other clinical skills relevant to role Awareness of accountability issues e.g. confidentiality Knowledge of situations and circumstances that can impact on patient safety and demonstrate understanding of how the healthcare support worker contributes to maintaining patient safety The ability to communicate effectively with patients and staff both written and orally and demonstrate a level of English language and grammar commensurate with writing in service user case notes

Must be able to meet the travel requirements of the post.
