First Choice Home Health & Hospice is hiring a Certified Occupational Therapy Assistant (COTA) to provide one-on-one occupational therapy services to home health patients.
This is a great opportunity for a COTA looking for flexible daytime work, professional autonomy, and meaningful patient care in the home setting.
Qualifications
Required:
• Current Utah Occupational Therapy Assistant license in good standing
• Daytime availability
• Valid driver's license
• Reliable transportation
• Strong communication and patient-care skills
• Ability to work independently in patients' homes and collaborate with the supervising Occupational Therapist
Preferred:
• Home health experience
• Adult or geriatric therapy experience
• Experience with neurological, orthopedic, or functional rehabilitation
What You'll Do
• Provide skilled occupational therapy treatments in patients' homes under the direction of the Occupational Therapist
• Carry out established treatment plans and goals
• Help patients improve activities of daily living, safety, mobility, and independence
• Educate patients and caregivers on home programs, adaptive techniques, and safety strategies
• Monitor patient response to treatment and communicate progress or concerns to the supervising OT
• Collaborate with nurses, therapists, physicians, and other members of the care team
• Complete accurate and timely clinical documentation

Patient visits are scheduled based on patient needs, with flexibility to organize your schedule throughout the week.
Territory: Salt Lake County Work Setting: Patient homes and residential care settings
