While we may not have an immediate position available, we encourage interested applicants to apply so we can consider them as opportunities arise. *Due to the nature of this case, applicants may be asked to attend a pre-hire shadow as a part of the hiring process* Responsibilities Provide direct patient care in accordance with individualized care plans. Administer medications and treatments as prescribed by the physician. Monitor patients' conditions and report any changes to the healthcare team. Assist with daily living activities, including personal hygiene and mobility support. Educate patients and their families on health management and wellness strategies. Maintain accurate electronic documentation of patient progress and interventions. Participate in ongoing training and professional development opportunities. May be required to travel up to a 30 mile radius. Qualifications Current LVN license in the state of California. Experience in Nursing Experience with Tracheostomies, G-tubes, and Suprapubic Catheters is preferred Ability to provide critical care for patients with complex needs.
