Main duties of the job The Healthcare Assistant role at HCRG Care Group involves delivering high-quality care to patients in their own homes and community clinics across Dartford and North Kent. Essential qualifications include an NVQ Level 3 in Health & Social Care or equivalent experience, alongside strong communication and organizational skills. A full UK driving license and access to a vehicle are also required.

This is a varied and rewarding role where you'll work alongside Community Nurses to provide compassionate, person-centred care that promotes independence, wellbeing and dignity.

Our teams are equipped with modern technology, enabling you to access patient records and complete documentation remotely, helping you spend more time delivering care and less time travelling back to base.

Located just off the M25, Livingstone Community Hospital benefits from excellent transport links and is within easy reach of Bluewater Shopping Centre and local amenities.

Main ResponsibilitiesWorking closely with Community Nurses and the wider multidisciplinary team, you will:

Deliver high-quality care to patients within their own homes and community clinic settings Support patients with a range of healthcare needs, including wound care, diabetes management and end-of-life care Undertake delegated clinical activities within your scope of competence Monitor and report changes in patients' conditions to registered clinicians Promote patient dignity, independence and wellbeing at all times Accurately document care provided using electronic patient record systems Develop positive working relationships with patients, families and healthcare professionals Contribute to preventing unnecessary hospital admissions through effective community-based care Support the continual improvement of services through shared learning and best practiceAbout YouWe're looking for a compassionate, motivated and organized individual who is passionate about delivering outstanding patient care.

To be successful in this role, you'll have:
