Job summary
We are recruiting to a permanent 8A Complex Care Manager. The complex care manager is a member of the Proactive Care - Mental Health Learning Disabilities and Autism team within the strategic commissioning directorate with responsibility for delivering all functions related to individually commissioned, jointly funded packages of care across NEL. This includes but is not limited to packages for children with special educational needs, adults with learning disabilities, neuro-diversity and mental health needs and other complex presentations, it does not include continuing healthcare. Many of these packages will be joint funded with the local authority.
The postholder will work closely as a team to build commissioning plans and pathways to enable a shift away from spot purchasing towards pathways of care which emphasise a move to living as independently as possible within the community, accessing universal provision where possible and reducing the risk of ongoing institutional care.The post holder(s) will report into the Head of Complex Care and the team will be supported by two co-ordinating roles.
Main duties of the job
The post holder will hold delegated responsibility for a defined complex care portfolio (inner or outer north east London, babies, children and young people (BYCP) or Adults), leading the commissioning, review and quality assurance of individual packages of care. Working within a system governance framework, the role will ensure that packages deliver high-quality, person centred outcomes, support de-escalation where appropriate, and represents value for money, while contributing to wider service and pathway development. The postholder will be responsible for ensuring the ICB is appropriately contributing to the development of Dynamic Support Registers and any successor arrangements. The team will be required to keep abreast of changing legislation and statutory guidance and to ensure their work is informed by NICE guidelines and by best practice. They will need to work closely with local government colleagues to build consistent and coherent joint commissioning of individual packages of care, with Place Delivery Teams and with the Heads of SEND co-ordination.

The remit includes (but is not limited to) the following:
Hold operational leadership for a defined portfolio of complex care, ensuring effective commissioning, review and assurance of individual packages of care. Hold expert understanding of the legal and policy requirements related to SEND, Learning Disability, neurodiversity and mental health packages of care depending on portfolio role. Act as the senior point of coordination for complex cases within the portfolio, ensuring timely decision making and resolution of quality, placement or cost issues. Manage, report and monitor CETR, CTR and other meetings and reviews within the timescales required. Some face-to-face meetings may require travelling out of the borough Be responsible for the Dynamic Support Register arrangements as appropriate Lead multi-agency decision making for complex cases including Local Area Emergency protocol (LAEP) meetings Take a population health management approach to understand current and future need Contribute to the development and implementation of commissioning and market management plans by providing operational insight form casework and provider partnerships. Manage relationships with Local Authorities, set up assurance meetings as required Oversee and maintain patient registers Lead on a KLOE tracker and report Maintain a tracker including a tracker for discharge plans which includes S117 applications, capacity assessments, risk assessments and barriers to discharge. Oversee the completion of Root Cause Analyses to identify where improvements to care can be made Maintain a view of spend against budget and developing plans to support better value for money Lead on improvement and CIP plans as required. Person Specification
Education / Qualifications Essential
Educated to masters degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area
Experience Essential
Experience and proven expertise in working with people (either children or adults) with complex needs requiring placements Experience in relevant disciplines e.g. project management, change management, finance, contracts, estates, communications Highly developed specialist experience of working to develop individual packages of care in either a health or social care environment with multiple partners
Knowledge Essential
Deep understanding of the public sector, particularly health and social care, including all relevant policy drivers A good understanding of governance, assurance and risk management in a high cost and high risk area
