Job summary

This post is open to experienced nurses, newly qualified professionals, and students who are due to complete their qualification imminently.

Freshney Pelham Care are a community nursing team within North East Lincolnshire providing nursing care and support to patients, families and carers with the aim of supporting patients to remain in their own homes, preventing hospital admission, and managing end of life care.

Our community nursing staff work closely in partnership with Primary Care Networks, Secondary care, Social Care, Specialist Nursing Teams and other healthcare staff to provide a high-quality service with care personalised to meet individual needs. The community nursing service supports and assists patients to manage ill health and chronic and complex conditions, allowing them to have the best quality of life whilst maintaining dignity and respect.

Main duties of the job

Role: Community Staff NurseHours: Full or Part timeInitially fixed term, with opportunity for a permanent position. Base: Freshney Green PCCHours: As a seven-day service, you will be required to work between 8am to 8pm, 7 days a week, 365 days a year on a rota basis. You will be required to work evenings, weekends and bank holidays.

We have the fantastic opportunity for a Community Staff Nurse to join our team. Whether you have previous experience or are newly qualified, this could be the role for you!

You will have the ability to maintain clear and accurate records, use information technology and will have a proven ability to work on your own initiative as well as part of a team. The post involves the day-to-day management of an unpredictable caseload, some of which will be complex in nature and therefore effective organisational and prioritisation skills are essential.

This job is based in the community, and you will be required to travel across North East Lincolnshire in your own vehicle. You will be required to meet deadlines and employ a flexible approach to your work and patient care.

Most importantly you must be able to demonstrate your commitment to the values of FPC.

As a small team, we provide community nursing services and work closely with Woodfield, Littlefield and Pelham GP Practices. Each surgery is represented on our Board of Directors by a nominated GP.

As a provider of health care, our commitment is to deliver services to the population which include nursing care, support and advice, signposting, and preventative services to enable independence for as long as possible.

We are based within the Freshney Green Primary Care Centre on Sorrell Road, Grimsby, and consistently strive to deliver a high-quality community nursing service for the GPs and their patients.

Person Specification

Qualifications Essential

Successful completion of Diploma or Degree in Nursing. Registered Nurse on NMC Register.

Desirable

Prepared to work towards Practice Supervisor and Assessor. Committed to continuous development through further study. Evidence of post registration education and training/competency portfolio.

Knowledge and Skills Essential

To work always within own competency. Effective communication skills verbal and written. Effective listening skills. Effective IT skills. Clear contemporaneous documentation written and computer recorded. Ability to prioritise work and plan accordingly. Expected to participate in the education and support of staff members. Ability to make decisions autonomously. Able to work with minimal supervision. Ability to provide research and evidence-based care.

Desirable

An understanding of primary care. IT skills - previous experience of working on EMIS systems, Microsoft Office and NHS email.

Experience Essential

Team working. Able to assess patients and carers holistically and implement care planning. Knowledge of strategic local plans, policies, sops. Can demonstrate attitude and aptitude appropriate to the post and ability to relate previous experience to the role. Extended experience of working in a healthcare setting

Desirable

Experience of working within a multi-disciplinary team. Experience of teaching and assessing students.
