Job summary

Grove Medical Centre is looking for an enthusiastic and experienced Practice Nurse to join our friendly and supportive team for 16 hours per week across 2-3 days. This is an excellent opportunity for a motivated nurse who is passionate about delivering high-quality patient care and enjoys working autonomously within a busy primary care setting.

You will play a key role in supporting the health and wellbeing of our diverse patient population through chronic disease management, cervical screening, immunisations, health promotion, wound care and preventative healthcare. You will also lead on Infection Prevention and Control, contributing to the ongoing quality and safety of our services.

Main duties of the job

As a Practice Nurse at Grove Medical Centre, you will provide high-quality, patient-centred nursing care across a range of clinical services within a busy and supportive primary care environment. The role offers a varied workload, including the management of long-term conditions, health promotion, cervical screening, immunisations, wound care and preventative healthcare.You will work closely with GPs and the wider multidisciplinary team to support patients in managing their health, promote wellbeing and deliver evidence-based care. The successful candidate will also lead on Infection Prevention and Control, helping to maintain high standards of quality, safety and regulatory compliance across the practice.

This role requires a nurse who is confident working both independently and as part of a team, with excellent communication and organisational skills. You will have the opportunity to build strong relationships with patients, contribute to service improvement initiatives and play an active role in supporting the health needs of our diverse local community.

The position is ideally suited to an experienced Practice Nurse or Community Nurse who enjoys delivering holistic care, supporting patients to achieve positive health outcomes and contributing to the ongoing development of primary care services.

Practice Nurse

About the RoleWe are looking for a compassionate, motivated and experienced Practice Nurse to join our friendly and supportive team at Grove Medical Centre.

This is a rewarding opportunity to play a key role in delivering high-quality care to a diverse patient population while working within a well-established and forward-thinking GP practice. As an integral member of our multidisciplinary team, you will provide a broad range of nursing services, including chronic disease management, cervical screening, immunisations, wound care and health promotion. You will also lead on Infection Prevention and Control, helping to ensure the practice maintains the highest standards of patient safety and quality care.

What You'll Be DoingYou will:

Deliver high-quality nursing care to patients across all age groups.Run clinics for long-term conditions including Diabetes, COPD and Asthma.Undertake NHS Health Checks, New Patient Health Checks and Learning Disability Health Checks.Provide cervical screening and women's health services.Deliver routine, childhood and travel vaccinations.Carry out wound care, dressings and suture or staple removal.Identify patients at risk of developing long-term conditions and support preventative care.Promote healthier lifestyles through education and health promotion activities.Lead on Infection Prevention and Control, including audits, training and annual reporting.Work collaboratively with GPs, pharmacists and the wider healthcare team to deliver safe, effective and evidence-based care.Support vulnerable patients through coordinated care and partnership working with external services.Contribute to quality improvement initiatives and service development across the practice.About YouYou will be a Registered Nurse with current NMC registration and experience working in primary care or community nursing. You will be confident working independently, have excellent clinical and communication skills, and be committed to delivering patient-centred care.

Experience in chronic disease management, immunisations and infection prevention and control is essential.

Qualifications in cervical cytology, family planning, teaching or chronic disease management would be advantageous.

Person Specification

Qualifications Essential

The successful candidate will demonstrate: Registered Nurse with a current and valid NMC registration. Experience working as a Practice Nurse, Community Nurse or in a Primary Care setting. Experience managing patients with long-term conditions, including Diabetes, COPD and Asthma. Experience delivering immunisations and vaccination programmes. Knowledge and practical experience of Infection Prevention and Control standards and procedures. Experience carrying out health promotion and preventative healthcare activities. Competent in wound care, dressings and removal of sutures and staples. Ability to work independently and make clinical decisions within scope of practice. Excellent communication and interpersonal skills with a patient-centred approach. Strong organisational and time management skills. Ability to maintain accurate and contemporaneous clinical records. Experience using clinical systems and electronic patient records. Understanding of safeguarding responsibilities and patient confidentiality. Ability to work effectively as part of a multidisciplinary team. Commitment to delivering high-quality, evidence-based care. Commitment to ongoing professional development and continuous learning.

Desirable

Experience working with EMIS Web. Cervical Cytology qualification. Family Planning qualification. Independent Prescribing qualification. Mentor, Assessor or Teaching qualification. Postgraduate qualification in Chronic Disease Management. Travel Health qualification and experience. Experience leading or supporting quality improvement initiatives. Experience acting as an Infection Prevention and Control Lead. Knowledge of QOF, Enhanced Services and NHS Primary Care contracts.
