Looking for a friendly, supportive team where you can practise high-quality medicine?

We would welcome a motivated, enthusiastic and dedicated GP to join us in our forward-thinking rural practice in beautiful Herefordshire.

We are supporting a valued team member to pursue a prestigious research post. To support them and optimise our services, we are extending and restructuring our current team to deliver our evolving improvement plan. By moving to Urgent Care Triage, we aim to enhance work-life balance with an end goal in line with the BMA guidelines, while further streamlining workflows to reduce administrative burdens

The Role

6-8 sessions per week Competitive salary (dependent on experience) Structured working day with manageable patient contacts Shared home visits 6 weeks annual leave + 1 week study leave (pro rata) Indemnity covered NHS Pension

Main duties of the job

We are looking for an approachable, collaborative colleague who thrives in a team-focused environment. The ideal candidate will bring a positive, supportive attitude to our team. You will undertake the usual range of duties including surgery face to face and telephone consultations, online triage, video consultations, home visits (minimal) and the associated administration including but not limited to reports, path lab results, documents and prescriptions in a timely manner.

Chronic disease management clinics are largely managed by our excellent team of NPs, nurses, pharmacy and health care team.

We offer minor surgery and sexual health enhanced services. We welcome specialist interests.

We have an efficient and well-run dispensary serving half of our patient population with experienced staff and led by our in-house Pharmacist. Our repeat prescribing is excellently managed by a senior pharmacist.

Job description Job responsibilities

The practice timetable is designed to ensure the optimum delivery of our services to patients and other contractual obligations and these will vary over time. We are currently implementing our second GP improvement programme moving to a GP triage system for urgent care.

In accordance with this, and at the present time, the postholder will undertake a variety of duties including, but not limited to,

Online consultation Triage

Telephone and face to face consultations and queries

Visiting patients at home and in the community hospital

Checking and signing repeat prescriptions and dealing with queries

Handling results, correspondence and associated administrative tasks in a timely fashion (paper based, electronic etc).

Participate and contribute to effective functioning of the practice team and its links to the wider communities

Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation

Assess the health care needs of patients with undifferentiated and undiagnosed problems

Screen patients for disease risk factors and early signs of illness

Develop care plans for health in consultation with patients and in line with current practice disease management protocols

Provide counselling and health education

Adhere to the clinical governance requirements of the practice

Adhere to the information governance and information management requirements of the practice including the Quality and Outcomes Framework.

Participate in and comply with the commissioning requirements of the practice

Prescribe in accordance with the practice prescribing guidelines

Attend clinical meetings/other meetings as required

Undertake other appropriate duties agreed between the practice and the employee.

In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
