Job summary
SS9-North Primary Care Network is seeking an experienced Emergency Care Practitioner (ECP) to join our multidisciplinary team, with a particular focus on Integrated Neighbourhood Team (INT) working.
The postholder will provide autonomous assessment, treatment and management of patients with urgent, acute and complex healthcare needs across primary care and community settings.
Alongside core ECP duties, the role will support closer working between primary care, community services, social care and voluntary sector partners, with a focus on proactive care, admission avoidance and improved coordination for patients with complex needs.
Main duties of the job
Clinical Practice
Undertake autonomous clinical assessment, diagnosis and management of patients presenting with acute and urgent healthcare needs. Carry out history taking, physical examination, clinical observations and appropriate treatment. Develop differential diagnoses and management plans within professional competence. Undertake home visits and support same-day and urgent primary care activity. Manage acute presentations and exacerbations of long-term conditions where appropriate. Identify clinical deterioration and escalate or refer appropriately. Maintain accurate and timely clinical records. Prescribe medicines where appropriately qualified and working within scope of practice.
Integrated Neighbourhood Team Focus
Act as a clinical link between SS9-North PCN and wider INT partners. Work collaboratively with primary care, community health services, social care, mental health, hospices, voluntary organisations and other local services. Identify patients who would benefit from coordinated multidisciplinary support. Contribute to MDT meetings, case discussions and care planning. Support proactive management of frailty, complex long-term conditions and patients at risk of hospital admission. Help improve communication and referral pathways between organisations. Support admission avoidance and appropriate community-based alternatives to hospital care. Promote personalised care that considers both clinical and wider social needs.

SS9-North Primary Care Network (PCN) is a collaborative network of three GP practices Eastwood Group Practice, Dr Krishnan and Partners, and The Leigh Surgery working together to improve access, enhance patient care and develop services for approximately 22,000 patients across the local area. The PCN employs a growing multidisciplinary workforce including Advanced Clinical Practitioners, Emergency Care Practitioners, pharmacists, care coordination and healthcare support roles. Proactive Care and Service Development
Support proactive identification of patients at risk of deterioration. Contribute to frailty, complex care, discharge follow-up and seasonal resilience initiatives. Help identify gaps in existing pathways and support practical service improvements. Participate in audit, quality improvement and evaluation of INT activity. Contribute clinical expertise to the development of new neighbourhood services and initiatives.
Multidisciplinary Working
The ECP will work closely with the wider PCN workforce, including GPs, ACPs, pharmacists, pharmacy technicians, FCPs, care coordinators, HCAs and other community clinicians.
The postholder will also develop strong relationships with wider INT partners to support coordinated care across the local neighbourhood.

Person Specification
Qualifications Essential
HCPC registered Paramedic Recognised Paramedic qualification (BSc/DipHE or equivalent) Relevant enhanced/advanced clinical practice qualification (Level 6 or 7 or equivalent) Current professional registration and evidence of ongoing CPD
Desirable
Independent Prescribing qualification. Experience within general practice Primary Care Networks or community services. Experience of home visiting, frailty or complex care. Experience of admission avoidance or urgent community response. Experience of integrated or multidisciplinary care. Knowledge of Integrated Neighbourhood Teams and proactive population health approaches. Experience of service development audit or quality improvement.
