Band 5 - Mortality & Clinical Harm Manager

Due to promotion an exciting opportunity has arisen at Band 5 level for a motivated, dynamic team player to join the Mortality & Clinical Harm Team in the role of Mortality & Clinical Harm Manager.

We are looking for someone with strong people skills, a positive attitude, and excellent time management and prioritisation skills, with the ability to plan ahead and meet tight deadlines without compromising standards or accuracy.

Main duties of the job

The Mortality and Clinical Harm Team forms part of the Quality Governance Team and is responsible for overseeing, developing and supporting mortality, clinical harm and Child Death Review processes across University Hospitals Plymouth NHS Trust. The team supports the scrutiny of mortality data, Structured Judgement Reviews, coronial activity and Child Death Reviews, using outputs to drive learning, quality improvement and patient safety across the organisation. The team also acts as a central source of expert advice and guidance for clinical and non-clinical colleagues.

The post holder will:

Lead the operational coordination of the Trust's Child Death Review process. Support the oversight and quality assurance of mortality review processes, including Structured Judgement Reviews and related governance activity. Analyse and report on mortality and child death review data to identify learning, themes and opportunities for improvement. Support compliance with national guidance, statutory requirements and local governance processes. Work collaboratively with clinical teams, governance leads and external partner organisations to improve patient safety and organisational learning. The role requires regular communication with staff at all levels of the organisation and with external organisations, frequently involving highly sensitive and confidential information.

Line manage and support ongoing development within the Mortality and Clinical Harm Team.

Job description Job responsibilities

1. Mortality Governance and Review Processes

Support the effective oversight, scrutiny and ongoing development of mortality and clinical harm review processes across the Trust. Maintain oversight of the Structured Judgement Review (SJR) process, ensuring reviews are completed, quality assured and recorded appropriately. Provide advice and support to clinical colleagues undertaking mortality reviews and Structured Judgement Reviews. Support the management and monitoring of coroner referrals and associated governance processes. Identify themes, trends and opportunities for learning arising from mortality reviews and support implementation of improvement actions. Ensure learning is identified, recorded, disseminated and monitored to improve the quality and safety of patient care.

2. Child Death Review Coordination

Coordinate the Child Death Review process and act as a key point of contact for Child Death Review activity alongside the named doctor for child death. Coordinate notifications, tracking and progression of child death cases through local and statutory review processes. Ensure compliance with statutory Child Death Review requirements, national guidance and agreed reporting timelines. Arrange, facilitate, minute and support Early Response Meetings, Child Death Review Meetings, mortality meetings and other multi-agency review forums. Liaise with Child Death Overview Panels (CDOPs), safeguarding teams, social care, police, education providers, Integrated Care Boards and other partner agencies. Monitor progress of actions arising from Child Death Reviews and ensure learning is appropriately disseminated and recorded. Develop and maintain effective working relationships with partner agencies to support collaborative review processes and organisational learning.

Person Specification

Knowledge & Experience Essential

Demonstrable experience of working in a complex, fast-paced environment across organisational boundaries. Demonstrable experience of coordinating complex processes involving multiple stakeholders. Demonstrable experience of using NHS information systems and incident management systems. Demonstrable experience of governance, compliance and assurance processes within a large and complex organisation. Demonstrable experience of analysing, interpreting and presenting data and information to support decision-making and quality improvement. Demonstrable experience of working with databases, digital systems and reporting tools. Understanding of patient safety, governance and quality improvement methodologies. Proven ability to review complex information from multiple sources and apply systematic decision-making.

Desirable

Demonstrable experience of working within the NHS Demonstrable experience of coordinating multi-agency meetings and review processes. Demonstrable experience of Board, Committee or governance reporting. Knowledge of national Child Death Review statutory guidance and Child Death Overview Panel processes. Knowledge of healthcare regulation and wider patient safety systems.

Aptitudes & Abilities Essential

Excellent written, verbal and presentation skills. Advanced Microsoft Office skills, particularly Excel, Word and PowerPoint. Proven ability to communicate complex and sensitive information effectively and appropriately to a wide range of audiences. Excellent organisational skills with proven ability to prioritise competing demands and work to tight deadlines. Proven ability to build and maintain effective working relationships with staff at all levels and external partner organisations. Strong analytical and problem-solving skills with the ability to identify themes, trends and improvement opportunities. Proven ability to interpret, critically analyse and present complex information. Proven ability to produce high-quality reports, briefing papers and presentations. High level of accuracy and attention to detail. Proven ability to deal appropriately with confidential and highly sensitive and distressing information.

Qualifications Essential

Educated to degree level in a relevant subject or equivalent level of knowledge and demonstrable experience gained through working at a similar level in a related field. Thorough knowledge of administrative procedures, governance processes and specialist software packages acquired through demonstrable experience or formal training. Evidence of continuing professional development.
