Main duties of the job

This role is key in delivering an exceptional, effective structured diabetes education service for patients across the UK (evening and weekend availability is required). Flexibility is required and offered.

Delivery of the EMPOWER programme to patientsDelivery of face-to-face courses within the advertised area and occasional video coursesEnsuring our EMPOWER programmes are delivered in accordance with the curriculum, syllabus, educator handbook, lesson plan and service(s) Operating ManualEnsure delivery of the EMPOWER programme in accordance with all Spirit policies and procedures

Why am I here? (Purpose)

The postholder will deliver diabetes education courses to patients referred to our service to enable them to live well with diabetes.

What am I aiming to achieve? (Outcomes)

The post holder undertakes training and passes competence assessment to deliver diabetes educationDiabetes education is delivered in an inclusive and interactive manner which allows for questions and discussion on the topics coveredDelivery of education is patient centred and allows for a variety of learning styles to support cognitive learningThe educator introduces and covers each topic following the curriculum closelyEducators reflect on the session, identify what went well, what went less well and what changes could be made.

How I will know I have succeeded? (Success Measures)

The postholder undertakes training on the curriculum and lesson plan, delivers courses, passes a competency assessment and is accreditedThe courses are delivered using the most up to date contentThe courses achieve a consistently high patient feedback scoreThe educator understands the administration and course organisation process undertaken by the administration teamThe educator ensures that attendance is recorded on the databaseThe educator responds promptly to administrator queries

What will I do? (Responsibilities)

Delivery of interactive structured diabetes education programmes in accordance with the set curriculum and lesson planUse a range of sound teaching and learning techniques to improve participants ability to self manage their diabetesEnsure the course is delivered using the most up to date programme contentUnderstand the administration and course organisation process undertaken by the administration teamEnsure all paperwork is completed, eg, attendance sheet, quality of life questionnaires, goal setting ad action plans and satisfaction surveys

Person Specification

Experience Essential

You are a clinical/non-clinical educator with experience in teaching/training adults. You are an enthusiastic self-starter, understand the importance of promoting self-care and an ability to support patients in achieving positive change and improved health outcomes. You work autonomously, are reliable, punctual and a confident communicator. You want to make a difference This role requires travel within the area to deliver face-to-face courses in local community settings so you must be located within the area, own a car and be in possession of UK driving licence.

Qualifications Essential

Experience of delivering health education Sound clinical knowledge and understanding of diabetes Excellent teamwork and communication skills
