Job summary

Honiton Surgery is seeking an experienced and motivated Advanced Nurse Practitioner (ANP) to join our friendly, forward-thinking Practice, serving approximately 18,500 patients across East Devon. As a well-established Practice and lead Practice within our Primary Care Network, we are committed to delivering high-quality, patient-centred care through innovation, collaboration and continuous service improvement.

This is an exciting opportunity for an autonomous clinician who is passionate about primary care and wishes to work within a highly supportive multidisciplinary team. You will play a key role in delivering same-day access services, leading acute minor illness clinics and managing patients presenting with acute, minor illness and undifferentiated conditions.

Salary will be offered between Band 7 and Band 8a equivalent, depending on experience, clinical capability and whether a recognised Advanced Clinical Practice qualification has been obtained.

Main duties of the job

As an Advanced Nurse Practitioner, you will:

Independently assess, diagnose and manage acute and minor illness presentations. Undertake advanced clinical assessment and clinical decision-making. Prescribe safely and effectively as an Independent Prescriber. Request, interpret and act upon appropriate investigations. Deliver both face-to-face and telephone consultations. Work collaboratively within the Practice Duty Team alongside GPs, Pharmacists, and Nursing colleagues. Contribute to quality improvement, clinical governance and service development initiatives.

Learning and development

The effective use of training and development is fundamental in ensuring that all staff are equipped with the appropriate skills, knowledge, attitude and competences to perform their role. All staff will be required to partake in, and complete mandatory training as directed. It is an expectation for this post-holder to assess their own learning needs and undertake learning as appropriate.

The post-holder will undertake mentorship for team members and disseminate learning and information gained to other team members to share good practice and inform others about current and future developments e.g., courses and conferences. The post-holder will provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning.

Collaborative working

All staff are to recognise the significance of collaborative working and understand their own role and scope and identify how this may develop over time. Staff are to prioritise their own workload and ensure effective time-management strategies are embedded within the culture of the team.

Teamwork is essential in multidisciplinary environments, and the post-holder is to work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working and to work effectively with others to clearly define values, direction and policies impacting upon care delivery

Effective communication is essential, and all staff must ensure they communicate in a manner which enables the sharing of information in an appropriate manner. All staff should delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence. Plans and outcomes by which to measure success should be agreed.

Managing information

All staff should use technology and appropriate software as an aid to management in the planning, implementation and monitoring of care and presenting and communicating information.

Data should be reviewed and processed using accurate SNOMED codes to ensure easy and accurate information retrieval for monitoring and audit processes.

Service delivery

Staff will be given detailed information during the induction process regarding policy and procedure.

The post holder must adhere to the information contained within the organisations policies and regional directives, ensuring protocols are always adhered to.

Primary key responsibilities

The following are the core responsibilities of the ANP in delivering health services:

Independently assess, diagnose and manage patients presenting with acute illness, minor illness and undifferentiated conditions. Deliver face-to-face and telephone same-day access appointments. Undertake advanced history taking and physical examination. Formulate differential diagnoses and management plans. Prescribe medicines safely and effectively as an Independent Prescriber. Request, interpret and act upon diagnostic investigations. Manage acute presentations and exacerbations of chronic disease within scope of practice. Work collaboratively as part of the multidisciplinary Duty Team. Refer patients appropriately to secondary care and community services. Identify, assess and appropriately manage clinical risk. Deliver patient-centred care based upon current evidence and best practice. Maintain accurate and contemporaneous clinical records using appropriate clinical systems and SNOMED coding. Support safeguarding processes in accordance with local and national guidance. Participate in clinical audit, quality improvement and service development. Contribute towards achievement of QOF and enhanced service requirements. Participate in significant event reviews and patient safety activities. Support the team in managing clinical emergencies. Undertake mandatory training and maintain professional registration and competencies. Contribute to and embrace clinical governance. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.

Wider responsibilities

In addition to the primary responsibilities, the ANP has the following wider responsibilities:

Provide clinical leadership and support to the multidisciplinary team.

Promote evidence-based practice and shared learning. Support service development and quality improvement initiatives. Contribute to the development of clinical protocols and pathways. Support workforce development and mentorship of colleagues where appropriate. Participate in local and PCN-wide initiatives to improve patient outcomes.

Person Specification

Qualifications Essential

Qualifications Qualified healthcare professional with current NMC registration. Educated to masters degree level in accordance with the NHS England. Multi-Professional Framework for Advanced Clinical Practice. Registered on the appropriate professional register. Independent Prescriber Qualification (V300). Evidence of ongoing Continuing Professional Development.

Desirable

Teaching, mentoring or supervision qualification Minor illness or urgent care qualification

Clinical Knowledge and Skills Essential

Advanced clinical assessment skills. Ability to independently assess, diagnose and manage acute and minor illness. Strong clinical reasoning and decision-making skills. Competent Independent Prescriber. Ability to request, interpret and act upon investigations. Understanding of evidence-based practice. Good clinical system knowledge and accurate record keeping. Understanding of safeguarding adults and children. Understanding of clinical governance and risk management. Knowledge of QOF and enhanced services.

Minimum two years' experience as a Nurse Practitioner or Advanced Nurse Practitioner. Demonstrable evidence of autonomous clinical practice. Experience assessing, diagnosing and managing acute and minor illness. Experience managing undifferentiated presentations. Strong communication and interpersonal skills. Ability to work independently and within a multidisciplinary team. Commitment to delivering patient-centred care. Knowledge and experience of acute and chronic disease management. Knowledge of safeguarding, clinical governance and risk management.

Desirable

Experience of Emergency Care, Urgent Treatment Centre or Minor Injury Unit settings. Experience of working in General Practice. Experience of same-day access or Duty Team working. Experience using Systmone.
