Wigton Medical Practice is looking to recruit an enthusiastic, conscientious and motivated Health Care Assistant/Care Co-ordinator to join our fantastic multi-disciplinary team. Ideally, you will hold hold an NVQ 3 qualification or equivalent or have proven experience in this role.

Working as part of a multi-disciplinary team, you will support the nursing team and deliver a range of clinical care within the scope of your role.

You will have a calm, confident and helpful manner with excellent communication skills along with the ability to problem solve and work under pressure. You will be educated to a good standard and must have excellent IT skills.

You will need to feel comfortable with technology, and also with change since the NHS is required to constantly change to meet the emerging needs of our population.

Main duties of the job

The Healthcare Assistant (HCA) / Care Coordinator is a hybrid role providing both clinical and administrative support to ensure patients receive safe, effective and timely care.

Within a clearly defined job role you will be able to work autonomously as well as assist other clinical sand admin staff in the provision of treatment, preventative care, health promotion and patient education.

The successful candidate will have proven experience in a wide range of clinical services including venepuncture, blood pressure monitoring, urinalysis and ECGs. In addition our Health Care Assistants also carry out injections and vaccinations against a specific schedule and support our Chronic Disease Monitoring. You will have a good working knowledge of infection control, safeguarding and the importance of clear and accurate record keeping.

Key Responsibilities

Clinical Healthcare Assistant Duties

Undertake delegated clinical duties in accordance with practice protocols, competencies and national guidance including:

NHS Health Checks. Blood pressure monitoring. Height, weight and BMI measurements. Venepuncture. ECG recording. Urinalysis. New patient health checks. Lifestyle advice including smoking cessation, weight management and physical activity. Removal of sutures and clips (where trained). Simple dressings. Point of care testing where applicable. Chaperoning clinicians. Preparing clinical rooms. Stock control and ordering of clinical supplies. Cleaning and maintaining clinical equipment. Infection Prevention and Control responsibilities. Care Coordination Duties

Support the proactive management of patient care by:

Monitoring recall systems for chronic disease management. Contacting patients to arrange reviews, investigations and monitoring. Supporting patients to access appropriate services. Supporting personalised care initiatives. Maintaining accurate care coordination records. Identifying patients requiring additional support or follow-up. Escalating concerns to clinicians appropriately. Long Term Condition Support

Assist with the management of patients with long-term conditions including:

Diabetes. Hypertension. Dementia Chronic Kidney Disease. Cardiovascular Disease. Responsibilities include:

Booking monitoring appointments. Identifying patients overdue reviews. Supporting annual review programmes. Ensuring required investigations are completed prior to clinician appointments. Supporting QOF and Enhanced Service achievement. Administrative Responsibilities

Maintain accurate patient records within EMIS. Process clinical documentation as directed. Support lead nurse with stock control Support lead nurse with infection control requirements Support recall and invitation systems. Assist with audit data collection. Participate in quality improvement projects. Maintain confidentiality at all times. Follow practice policies and standard operating procedures.

Experience Essential

* Must have worked in general practice in HCA or Phlebotomist role *Able to carry out venepuncture *Able to carry out blood pressure monitoring *Able to carry out urinalysis *Able to carry out routine physiological measurements *Able to carry out ECG's *Chronic Disease Management *Knowledge of Confidentiality and Safeguarding *Excellent IT skills *Excellent communication skills

Desirable

*Able to administer injections *Able to carry out diabetic foot checks *Able to assist in minor surgery / women's health clinics

Qualifications Essential

NVQ 2 or 3 in Health and Social Care * 4 GCSE's (or equivalent) which must include English and Maths at Grade C/Level 5 or above. Qualified in Phlebotomy

Knowledge Essential

Infection Prevention and Control Confidentiality and Information Governance Safeguarding Adults and Children Long term condition management NHS screening programmes
