Main duties of the job
The Extended Access Practice Nurse & Clinical Lead is responsible for delivering high-quality, safe and effective nursing care across the Black Country GP Hub, serving Citrus Health PCN and Newcomen PCN. The role combines direct patient care with clinical leadership, supporting Extended Access services during evenings, weekends and bank holidays.
Working alongside GPs, ANPs and the wider multidisciplinary team, the post holder will undertake nursing assessments, provide evidence-based treatment, support the management of long-term conditions, promote health and wellbeing, and contribute to achieving Quality and Outcomes Framework (QOF) targets. The role also includes providing specialist advice and education to patients and colleagues in areas such as COPD, Asthma and, where qualified, Diabetes care.
As Clinical Lead Nurse, the post holder will be the first point of contact for nursing staff within the service, providing professional leadership, supporting workforce development, promoting clinical governance, and ensuring high standards of patient care, safety and compliance with NMC and organisational standards. The successful candidate will be an experienced Primary Care Nurse with strong leadership, clinical and organisational skills, committed to continuous improvement and excellent patient outcomes.

Citrus Health PCN and Newcomen PCN work collaboratively to deliver a well-established Extended Access Service for 11 GP practices across the Black Country, providing healthcare services to a combined patient population of approximately 94,780 patients. The service operates through a central hub model and offers evening and weekend appointments, improving access to primary care outside of core practice hours. This collaborative approach has been in place since 2017 and enables patients to access a wide range of appointments closer to home.
Our multidisciplinary team includes GPs, Practice Nurses, Advanced Nurse Practitioners, Healthcare Assistants, Clinical Pharmacists, Pharmacy Technicians, Social Prescribers, Care Co-ordinators, First Contact Physios and administrative staff who work together to deliver high-quality, patient-centred care. The Extended Access Practice Nurse & Clinical Lead is responsible for delivering high-quality, safe, and effective nursing care across the Black Country GP Hub, serving the populations of Citrus Health PCN and Newcomen PCN. The post holder will combine direct patient care responsibilities with clinical leadership duties, supporting the delivery of extended access services across evenings, weekends, and bank holidays.
As the Clinical Lead Nurse, the post holder will be the first point of contact for Practice Nurses, Advanced Nurse Practitioners (ANPs), and Healthcare Assistants (HCAs) working within the Extended Access Service. They will work closely with the Hub Management Team, GP Clinical Leads, and PCN leadership teams to ensure contractual requirements, quality standards, governance arrangements, and patient outcomes are consistently achieved.

Clinical Practice
The post holder will:
Assess, plan, implement and evaluate programmes of care to promote health and wellbeing. Deliver evidence-based nursing interventions in line with national and local guidance. Manage and review patients with long-term conditions, supporting self-management and personalised care planning. Identify patients at risk of developing long-term conditions and initiate appropriate interventions. Provide health promotion, screening and preventative care to improve patient outcomes. Undertake opportunistic health promotion during patient consultations. Deliver cervical cytology screening and women's health services. Offer family planning advice and support. Assess, manage and treat uncomplicated wounds. Deliver childhood and adult immunisation programmes. Provide travel health advice and vaccinations where appropriate. Recognise, assess and refer patients presenting with mental health concerns. Provide advice regarding prescribed and over-the-counter medications. Maintain accurate, contemporaneous, and legally compliant clinical records. Work within professional competence and NMC standards at all times.Clinical Leadership
The post holder will:
Provide visible clinical leadership and professional support to Practice Nurses, ANPs, and HCAs working within the Extended Access Service. Act as the first point of escalation for clinical and operational nursing queries. Lead by example, promoting high standards of patient-centred care. Support workforce planning and service development initiatives. Ensure nursing staff work within professional standards, clinical policies and evidence-based practice. Promote a culture of continuous quality improvement and shared learning. Contribute to service planning and the achievement of contractual performance targets. Support recruitment, induction, supervision and retention of nursing staff. Facilitate team development and encourage collaborative multidisciplinary working.Clinical Governance & Quality
The post holder will:
Ensure all clinical activity is delivered in accordance with NMC standards, NICE guidance, local policies, and national frameworks. Maintain awareness of regulatory updates and implement changes where required. Review and update clinical protocols and operating procedures. Participate in audits and quality improvement initiatives. Monitor service performance and support action planning where improvements are identified. Contribute to the investigation of complaints, incidents, significant events and near misses. Undertake root cause analysis and support implementation of learning outcomes. Promote patient safety, clinical effectiveness, and continuous quality improvement. Support safeguarding processes for children and vulnerable adults. Work within organisational policies relating to domestic abuse, substance misuse, and vulnerable groups.Infection Prevention & Control

Ensure compliance with all Infection Prevention and Control policies. Identify infection control risks and implement corrective actions where required. Promote safe infection prevention practices across all Extended Access clinics. Support monitoring and audit of infection prevention standards.Communication & Patient Engagement

Person Specification
Qualifications Essential
Registered Nurse with active NMC registration. Significant experience within Primary Care or General Practice Nursing. Experience managing patients with long-term conditions. Experience undertaking cervical cytology. Experience delivering immunisation and vaccination programmes. Demonstrable leadership or supervisory experience within a clinical setting. Excellent communication and organisational skills. Knowledge of clinical governance and quality improvement processes. Understanding of safeguarding and patient safety requirements. Ability to work flexibly across extended access hours.
Desirable
Experience leading or managing a small clinical team. Mentorship, teaching or assessor qualification. Experience working within PCN, Hub, or Extended Access services. Experience undertaking clinical audits and quality improvement projects. Experience in managing long term conditions. Qualifications in diabetes, respiratory disease, travel health, women's health or other long-term condition management. Strong interest in service development and clinical governance.
