Person Specification
Qualifications Essential
Previous experience working in a healthcare, primary care, social care or administrative environment. Good understanding of the needs of patients with long-term conditions, complex needs, older people and/or vulnerable patients. Excellent communication and interpersonal skills, with the ability to communicate effectively with patients, carers and healthcare professionals. Strong organisational and time-management skills, with the ability to prioritise workload effectively. Ability to work independently while also contributing effectively as part of a multidisciplinary team. Good IT skills and confidence using electronic systems and databases. Ability to maintain accurate, confidential and up-to-date patient records. Understanding of confidentiality, information governance and data protection requirements. Ability to demonstrate empathy, patience and a non-judgemental approach when supporting patients. Commitment to equality, diversity and treating patients with dignity and respect.
Desirable
Desirable NVQ Level 2/3 in Health and Social Care, Business Administration, Care or a related subject. Experience working within a GP practice, NHS, community healthcare or social care setting. Knowledge of local health, social care and voluntary-sector services. Experience of care planning, social prescribing, signposting or coordinating support for patients. Experience working with multidisciplinary teams. Knowledge of EMIS Web, SystmOne or other primary-care clinical systems. Experience supporting patients with multiple or complex needs. Understanding of safeguarding procedures and referral pathways. Personal Attributes Caring, approachable and patient-focused. Reliable, professional and well organised.
