What you’ll bring as Hospice Bereavement Coordinator?

• Coordinates the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family and caregivers. • Provides spiritual support and/or counsel to patients, families, and staff consistent with patient and families’ belief systems. • Plans bereavement program development, expansion, and refinement through annual evaluation of services. • Provides bereavement support to hospice personnel coping with work-related grief through one-to-one counseling, and supervision of interns providing one-to-one counseling. • Assists in evaluating and planning services to meet hospice personnel support. • Functions independently but seek and accepts guidance from other members of the interdisciplinary group, or from a member of the community. • Contacts clergy or appropriate representatives of patients or families as indicated.

What is Required?

• bachelor’s degree in psychology, social work, sociology, or related fields; or 8 years’ relevant experience. • 1 year of relevant experience in a counselor role in a Home Health service or Hospice environment is required • Experience in family therapy regarding loss, grief, death and dying. • Excellent organization, analytical, decision-making and communication skills • Able and willing to travel up to 50% as needed to support business and patient needs. • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment • Valid driver’s license and insurance and reliable transportation to perform job tasks

You will report to the Clinical Team Manager.
